Employee stated that he was getting out of his mechanic's truck. As he stepped onto the ground, he stepped onto a ridge of dirt and sprained his right ankle.

Employee stated that he had started up a step ladder to work on a front end loader but stepped back off to get a tool. As he stepped off, his right foot stepped onto uneven ground causing him to sprain his right ankle. No lost time incurred but reportable due to restricted work duties.

Employee stated that he was getting onto a dozer. He had one foot on the step of the push beam and the other foot on top of the push beam. His right foot slipped off of the step causing him to fall down and strike his right rib cage against the top of the push beam resulting in a fractured right rib.

Employee stated that he was getting out of the dozer. He stepped on a loose bolt on the deck causing him to slip. He grabbed hold of a safety rail to keep from falling. As he did, he strained his shoulder. Taken off work on 4-9-09 to have minor shoulder surgery to repair rotator cuff tear.

Employee stated he was getting onto dozer when left foot slipped off step. He had hold of grab iron which prevented fall but it strained right shoulder. Hurting but seemed ok. Went to doctor later. Found small tear in shoulder. Opted to keep working. Finally decided on minor surgery to repair tear. Did so and lost time starting on 11/17/08, thus reportable.

EE WAS DESCENDING THE LADDER ON THE BACK OF THE OIL TRUCK AFTER HE HAD FILLED IT WITH OIL. HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE LADDER & HE CREELED HIS ANKLE ON THE GROUND.

EE STATED THAT HE WAS CLIMBING UP THE HAULER STEPS TO CHECK THE OIL LEVEL AND TO START UP THE HAULER TO WARM IT UP. AS HE WAS CLIMBING THE STEPSHE MISSED THE HAND RAIL AND FELL BACKEWARDS AND ONTO THE GROUND(APPROX FALL OF 7 TO 8 FT.) CAUSING A COMPRESSION FRACTURE TO THE T12 VERTEBRAE.

EE STATED HE WAS OPERATING A DOZER ON 2ND SHIFT HE STOPPED THE DOZER TO CHECK AN OUTSIDE LITE WHICH HAD STOPPED WORKING AS HE WAS STANDING ON THE DOZER TRACK TO CHECK THE LIGHT, HE FELL OFF OF THE DOZER IN BETWEEN THE PUSH BEAM & ROLLER FRAME OF THE DOZER. MISSED 1ST WORK SHIFT ON 10/19/00
