EMPLOYEE HAD BURNED HOLES IN TRUCK BED AND WENT DOWN OFF TRUCK TO GET A LADDER AND BOLTS TO FINISH JOB.AFTER CLIMBING UP THE SIDE OF THE TRUCK BED WITH THE LADDER IN ONE HAND,THE EMPLOYEE STEP PED OVER THE TOP OF THE TRUCK BED AND HIS FOOT SLIPPED AND HE FELL TO THE BOTTOM OF THE TRUCK BED.
