EMPLOYEE WAS GOING TO SERVICE HIS MACHINE. AS HE STEPPED DOWN FROM THE MACHINE HE MIS-JUDGED THE DISTANCE TO THE GROUND. HE LOST HIS FOOTING AND FELL HITTING HIS FACE ON A PROTRUDING PIPE.
