While miner was waiting for polymer to finish transferring from trailer to stationary pre treatment tank, miner turned to the right and miner's feet slipped in the wet mud causing miner to fall and fracture right leg. The wet, muddy, red clay made very slippery conditions.

Showing all 22Employee stepped onto a berm to observe the cut conditions. The injured employee turned to walk down the berm and stepped into a low spot in the berm. The injured employee lost balance after stepping in the low spot and fell to the ground. The result of the fall was a dislocated right ankle and a broken right fibula bone.
While attempting to step across the hood opening on 728 end dump, the employee stepped short and fell on EE's right side, mid-torso. The fall resulted in two broken ribs.
The employee lost grip on the left grab iron before foot was on the ground resulting in a fall backward. The employee's head hit the bottom part of the ripper frame, which cut the top of head.
EE fell in restroom, hit their head on a cabinet and caused a bump and minor cut that stopped bleeding without a bandage. As a precautionary measure, 911 was called. Due to personal medical reasons, the physician stated that they possibly suffered a seizure which caused the fall. This Report was requested to be filed by MSHA inspector and is under protest by this contractor.
Worker was climbing in the back of EE's truck to get a bottle of water during break. The truck was pulling a trailer, and when the worker stepped down from truck EE's foot got caught on trailer causing worker to fall back, landing on EE's hand/wrist.
Welder was cutting a piece of plywood. Setting it down on the handrails of a walkway to draw it out, he stepped around the handrails and stepped off the walkway. His leg hit the grating on the other walkway cutting his leg that required stitches to close it up.

The employee had stopped his truck to use the porta-john. As the employee was climbing down the steps his foot slipped off and he turned sideways and struck the top of his left forearm on the truck platform. He did not use the top step but went straight to the bottom step. He was placed on restrictive duty with a bruise to the top of his left forearm.
Employee dismounting equipment, his foot slipped off muddy step and struck left Knee on track. First aid given at job trailer. Knee x-ray at occupational clinic morning of 1/25/12,bone fragment seen at knee cap. Employee put on crutches with brace and released with restrictions until seen by orthopedic doctor.
Employee was walking to supervisors truck at the asphalt area when he tripped over dozer tracks and hyperexteded his left knee causing swelling in the knee area. Employee was taken to the hospital emergency room where they drained his knee, gave him a pain injection, placed him on crutches and took him of work.
EE stuck his truck in the fill area. In helping the dozer operator hook the tow cable he twisted his left knee.

Employee was setting up to replace removable floor in transformer room in Dragline. Employee fell through the floor opening when trying to reach the shackle that was located above the floor opening breaking his wrist.

Mounted bulldozer to change fuel filters. The bulldozer tracks were muddy and the oiler slipped and fell off dozer track to the ground.

EMPLOYEE WAS SWEEPING THE SHOP FLLOR AROUND A DOZER BLADE THAT HAD BEEN REMOVED FROM THE DOZER. HE STEPPED OVER THE PUSH ARM OF THE DOZER BLADE AND TWISTED HIS KNEE. HE STEPPED ON DIRT CLODS I N THE FLOOR CAUSING THE INJURY.
EE WAS CLIMBING DOWN FROM TRUCK & MISSED THELASTLADDER RUNG & FELL APPROX 3 FT TO GROUND
