Employee was walking across section doing pre shift exam when he stepped in a hole and tripped and twisted his right knee. Employee worked until 8/12/14. then went off work.

removed wet end of pump for rebuilding and was moving to drill tender. Stepped on power cable, lost footing and smashed finger between tender and wet end of pump.
He was getting out of scoop deck & fell, landing on his side.
EMPLOYEE HAD MUD ON HIS BOOTS WHILE STEPPING INTO A TRACK MANBUS & HIS FEET SLIPPED ON THE METAL. THIS CAUSED HIM TO FALL BACKWARDS & STRIKE HIS BACK ON THE MANBUS.
THE EE WAS UNLOADING BOLTS, PLATES, GLUE ON A FLAT CAR & TRIPPED & TRIED TO CATCH BALANCE AND FRACTURED LEFT WRIST.
THE EE WAS CHANGING HOSE ON DIESEL SCOOP, HE STEPPED OFF OF SCOOP AND FELL BACKWARDS INTO BELT WINDER CAUSING HIM TO BUMP LEFT FOREARM. THIS INJURY WAS REPORTED TO SAFETY DEPT ON 4/3/00.
