EE WAS WALKING AROUND MINE OFFICE BLDG WHERE HE WOULD RETURN EXTRA SUPPLIES AND REMOVE HIS MINING APPRATUS I.E. BOOTS LIGHT HAT ETC. HE STEPPED ONTO A SMALL PALLET USED AS A BRIDGE THAT HAD A BOARD MISSING AND HIS FOOT WENT THRU CAUSING HIMTO LOSE HIS BALANCE AND FALL DOWN.
