An employee lost EE's grip while attempting to go up the steps of the CAT 980M Coal Loader. EE's right hand slipped off of the hand hold causing EE to fall backwards and break EE's left wrist.

Showing all 4An employee lost footing while going down the set of steps from the 3rd floor MCC room to the 3rd floor of the plant. The fall resulted in a strained left shoulder that required surgery on 11/22/19.
Employee was loading cap wedges in the stockpile area onto a forklift when he tripped over a metal band that held the cap wedges together for shipment to the mine. He fell catching his fall with his right hand and knee. On 6-29-12 he was put on restricted duty due to his wrist hurting and returned to full work duty on 7-9-2012--a total of six days.
Employee walked around a trough to unhook a chain fall and stepped up onto a piece of pipe to release hook, foot slipped and twisted leg.
