Employee was dismounting #88270 Prell Truck and his foot slipped causing his left hand/wrist to jam into the platform. Employee went to Scott Orthopedic and was examined by Dr. An x-ray showed a fracture of the fifth metacarpal on his little finger of his left hand. A cast was placed on his left hand and he was released to return to work full duty.

KNEE ON A ROCK FRACTURING THE PATELLA. HE ALSO SPRAINED HIS LT.SHOULDER & BOTH ELBOWS IN ARRESTING THE FALL. HE SOUGHT TREATMENT AT 5:00PM.
WHILE WALKING AROUND DOZER IN PM YARD, EMPLOYEE STEPPED IN AN OLD DRILL HOLE THAT HAD BECOME FILLED WITH WATER.
AS EE WAS DISMOUNTING DOZER HIS FOOT MISSED THE PUSH ARM STEP CAUSING HIM TO TWIST HIS (L) KNEE. EE FINISHED SHIFT AND WORKED THE NEXT SEVERAL DAYS. NOTE: EE WAS NOT ALLOWED TO RETURN TO WORK AFTER FOLLOW UP WITH DR ON MAY 27.
EE WAS PULLING ON TRAILING CABLE WITH A SHORT PIECE OF ROPE, WHEN THE ROPE BROKE CAUSING HIM TO LOOSE HIS BALANCE. HE SLIPPED & FELL, LANDING ON HIS RT. WRIST.
AS EMPLOYEE DISMOUNTED DRAGLINE LADDER HE STEPPED ON UNEVEN SPOT IN GROUND (CAUSED BY INDENTATION OF TUB) AND TWISTED HIS LEFT KNEE.

WHILE STANDING BESIDE HIS TRUCK, THIS EE TURNED INTENDING TO GET INTO HIS TRUCK. HIS FOOT STUCK IN MUD RESULTING IN PAIN TO HIS KNEE. ON 4/25/00 HE WENT TO A DR WHO DIAGNOSED TORN CARTILEGE IN HIS KNEE. HE HAD SURGERY ON 5/1/00 AND IS MISSING WORK.
EE SLIPPED ON ICE WHILE CLIMBING ONTO A FUEL TRUCK. HIS RIGHT FOOT WAS ON BOTTOM RUNG OF LADDER. HIS LEFT FOOT SLID ON ICE. HE FELL, WITH HIS RIGHT FOOT HUNG IN THE LADDER. HE WAS DIAGNOSED WI TH A TORN HAMSTRING MUSCLE, RESULTING IN LOST TIME.
