The employee was walking down the travel way of the Slope Belt. EE slipped and fell against the belt causing an abrasion to hand. The ee was dismounting a dozer. While maintaining 3 points of contact, ee strained shoulder when used arm to put weight on hand rail. The employee was attempting to climb the ladder in to a 777 truck at the start of the shift. He fell backwards while trying to get on the first step and injured his back. The employee was carrying a bag of rock dust on his shoulder when he stepped on an uneven section of mine floor. This caused him to fall and injure his knee. Employee was walking down a set of steps while maintaining 3 points of contact. The miner missed the last step in the set and fell forward striking his side on a hand rail. The miner was attempting to fill a pod duster when the fill hose lost pressure. When the hose re-gained pressure the employee stumbled and fell against the coal rib and hurt his ribs. The accident/ injury was not the result of compliance with rules or regulations, mining equipment or systems, skills, proficiency, training, attitude, protective items related to clothing or protective devises. The miner was walking and was not concentrating on the task, stepped in a hole and sprained his lower back.
The employee was walking up a covered staircase, that leads to the hourly parking lot. The employee's foot slipped off the front of a step which caused him to stumble onto that foot awkwardly. The incident resulted in a fracture to a metatarsal bone in the employee's foot. No defects were found during an inspection of the stairwell.
Employee was walking thru mud and slop at the HG side of the face, he tripped or slipped and put out his hand to catch himself, injuring his thumb. Cause of this injury were the mud and paying attention while walking thru this condition. Compliance/rules not a factor, miner proficiency may be a factor, mining/equip/systems is not a factor, PPE not a factor.
Employee was walking from the track entry to the belt entry in a crosscut and fell down landing on his shoulder. Began losing time on 1/21/2014.

EE slipped on 777 Cat Haul Truck. He lost his footing and struck his knee.
