Employee was stepping down off a 1 foot high platform and tripped over a hose. Employee landed on floor striking his left knee, left hip, and left shoulder. The employee has been diagnosed with a tear in his labrum and a partial rotator cuff injury.

Employee slipped and fell while walking on snow/ice covered walkway. Employee complained of left shoulder, and left knee pain. Employee notified employer of fall on 2-1-2013. Employee declined medical attention at that time. Employee sought medical advice in April of 2014. Employee had 3rd physical therapy appointment on May 14, 2014. This accident is now reportable.

EE was climbing down off of a coal screen. He extended his legs over side and was unable to reach floor. His feet were approx.6" from touching. He let himself drop the 6". His left leg touched first. When this occurred he felt a sharp pain and a pop in his left knee. He stated there was no slip/trip hazard. An examination by a physician found he had a medial meniscus tear.

Employee was unhooking tow cable attached to vehicle that was stuck on refuse pile. His left foot became stuck in the mud. When he pulled his left foot up to free it from the mud he twisted his left leg. Initial nature of injury thought to be a strain/sprain.

EE was on site to clean underground piping with a water jet via an open manhole. A hose reel guide was installed overtop of the open manhole to allow the hose to move freely. EE was standing on top of the hose reel guide when it moved him and the guide fell into the open manhole. EE caught himself on the sides of the manhole, causing contusions to both arms and left shoulder.

Employee was crawling out of a vibrating screen to the concrete floor. He stepped into the vibrating screen underflow pan and lost his balance. To keep from falling, he grabbed something with his right hand. He felt pain in his right arm/shoulder.

Employee was operating a Volvo A40D articulated hauler (2001). He was climbing the steps of the machine in order to clean the windows. Employee grabbed onto the handrail, while climbing, when the handrail broke off at the base, causing employee to fall to the ground.

Employee was standing inside a distribution box attempting to install a pipe inside the box. While positioning the pipe using both hands, his foot slipped causing him to twist his left knee. This resulted in a torn menicus in his left knee.

EMPLOYEE WAS OPERATING A BOBCAT SKID LOADER, CLEANING THE FLOOR IN THE VEHICLE MAINTENANCE BUILDING. UPON COMPLETION OF THIS TASK, HE PARKED THE BOBCAT. WHEN HE EXITED THE MACHINE, HE FELT SEVERE PAIN IN HIS LEFT KNEE.
