Showing all 12Employee stated he was working with a crew in the process of moving the mining equipment power cables up two crosscuts. His feet became entangled in a roof bolter machine cable loop causing the employee to fall causing pain to his right side. Employee started missing work on 11-6-09.
EE stated he was walking down slope. Slipped and fell, causing pain in lower back. Started missing work on 7/31/09

Employee stated he was walking underground, started to crossover shuttle car cable, tripped over shuttle car cable, causing employee to fall onto his left arm, causing fracture to lower left arm.
Employee was dismounting 773 ir bobcat...slipped and fell on his right wrist causing fracture to wrist...fracture required surgical attention by placing three steel pins in ee's wrist
EMPLOYEE STATED HE WAS WALKING DOWN THE #3 RETURN SLOPE ENTRY SLIPPED AND TWISTED LEFT KNEE CAUSING A POSSIBLE STRAIN TO HIS KNEE.
