OPERATOR CLIMBED ON TOP OF THE ROOF BOLTER TO GET SOME PINS HE COULD NOT REACH. HE JUMPED OFF THE ROOF BOLTER, LOST HIS FOOTING AND FELL ON HIS BACK.
