Around 9:30 pm EE stepped in a hole in the inby intersection of the #5 entry and twisted his knee. He finished the shift but as the shift progressed, the swelling got worse and so did the pain.
The injured employee is a belt repair man. He was standing on bottom belt putting a 100' piece of belt in on top with a set of channel lock pliers. He lost grip on belt with pliers and fell back- ward off belt striking a 4' water line with his back when landing.
Said putting a piece of rIb roll on belt. Twisted back, slipped and fell in mud.
The bottom was uneven where employee was walking stepped in a small hole twisted his rt ankle.
EMPLOYEE WAS CLEANING AROUND #8 BELT HEAD THAT WAS GOBBED OFF AND TRIPPED OVER TURNBUCKLE AND FELL TWISTING BACK AND LANDING ON LEFT SHOULDER & ARM. WENT TO HOSPITAL - TURNED IN A PREVIOUS CLAIM ON BACK & SHOULDER, AGGREVATING PREVIOUS INJURY.
EE SAID HE SLIPPED WHEN TIGHTENING SADDLE BOLTS ON SHUTTLE CAR AND HE HIT HIS LEFT SIDE (RIBS) AGAINST THE SHUTTLE CAR.
Employee was getting bottle of oxygen and stepped in hole & twisted left knee while helping to replace wheel unit on #3 s/car.

EMPLOYEE WAS GETTING INTO SCOOP AND HIT RIBS ON LEFT SIDE OF SCOOP CANOPY BRUISING HIS RIBS. HE WORKED WED AND THURS AND RIBS WAS BOTHERING HIM AND HE WENT TO DR. EE WAS OFF FRIDAY AND SAT , REPORTED BACK 10-20-03.
EE WAS WALKING AROUNT RT MINER AND SLIPPED ON SLICK BOTTOM AND FELL ON HIS SELF RESCUER - CAUSED INJURY TO HIS BACK WHEN HE LANDED ON IT. EE WENT TO HIS REGULAR DOCTOR FOR SOMETHING FOR PAIN - WAS GOING TO DO AN MRI. REPORTED 8-2-03.

EE WAS CUTTING A PIECE OF EXPANDED METAL OFF AT #4 BELT HEAD THAT WAS STICKING OUT IN WALKWAY. A SPLICE IN BELT CAME BY HIT EXPANDED METAL, EXPANDED METAL HIT EE'S RIGHT LEG ABOVE KNEE. EE ST EPPED BACK AND FELL OVER WATER LINE, MAY HAVE TWISTED RIGHT KNEE, IT WAS SWOLLEN A LITTLE.
EE WAS MOVING A REMOTE CONTROL MINER, WHILE WALKING BEHIND IT, EE STOPPED TURNED AROUND TO CHECK SLACK HE WAS PULLING AND STEPPED IN A POTHOLE. LEFT KNEE POPPED. THEN LATER ON IN SHIFT WHILE O N HIS KNEES, LEFT KNEE POPPED SEVERAL MORE TIMES.
EE SLIPPED WHILE GETTING OUT OF SCOOP DECK (LOST HIS BALANCE) TRIED TO CATCH HIMSELF WITH HIS LEFT HAND, EE LEFT HAND HIT GROUND FIRST THUMB OR HAND SPRAINED BACKWARDS CAUSING PAIN.
EE SLIPPED & FELL WHILE CARRYING A BAG OF ROCK DUST. EE SAID WHEN HE FELL HE LANDED ON HIS SELF CONTAINED SELF-RESCUER THAT IS ON HIS BELT.
