Hanging face curtain in #2 entry on 4ft ladder. Ladder leg buckled and employee fell from the 3rd rung on the ladder landing on elbow forcing shoulder upward.
Bolt man went to walk past the bolter after hanging his curtain and tripped and fell into bolt machine.
Traveling the face area, ice between 3 & 4 entry slight grade covered with ice. Employee went through fly pad slipped on ice landing on his back on his SCSR.
Slipped getting out of 602 Scoop and straddled seat and frame scrape in groin area.
While holding cables to hand up, stepped off small ledge, left knee bent backwards.
He was getting out of shuttle car and tripped over his own two feet, landing on his left wrist.
Working on power center - feet slipped causing him to hit his ribs against the power center - did not think he had initial injury - went to hospital on 3/21/10.

Injured was dragging a dust hose from a track mounted tank duster over & thru a crosscut to #9 belt head to start dusting on #9 belt. He stepped on an uneven ledge in the bottom, turning his right ankle resulting in a broken rt ankle.
Around 9:30 pm EE stepped in a hole in the inby intersection of the #5 entry and twisted his knee. He finished the shift but as the shift progressed, the swelling got worse and so did the pain.
The injured employee is a belt repair man. He was standing on bottom belt putting a 100' piece of belt in on top with a set of channel lock pliers. He lost grip on belt with pliers and fell back- ward off belt striking a 4' water line with his back when landing.
Said putting a piece of rIb roll on belt. Twisted back, slipped and fell in mud.
The bottom was uneven where employee was walking stepped in a small hole twisted his rt ankle.
EMPLOYEE WAS CLEANING AROUND #8 BELT HEAD THAT WAS GOBBED OFF AND TRIPPED OVER TURNBUCKLE AND FELL TWISTING BACK AND LANDING ON LEFT SHOULDER & ARM. WENT TO HOSPITAL - TURNED IN A PREVIOUS CLAIM ON BACK & SHOULDER, AGGREVATING PREVIOUS INJURY.
EE SAID HE SLIPPED WHEN TIGHTENING SADDLE BOLTS ON SHUTTLE CAR AND HE HIT HIS LEFT SIDE (RIBS) AGAINST THE SHUTTLE CAR.
Employee was getting bottle of oxygen and stepped in hole & twisted left knee while helping to replace wheel unit on #3 s/car.
According to worker's compensation, employee stepped on a crib block which turned over and he fell forward on his arm and knee.
Walking toward #9 heading employee's leg went from under him, causing him to fall. He hit his knee on a rock.

Ee was walking beside feeder in mud and fell into feeder striking his shoulder - went to hospital - had severe shoulder strain.
EE WAS BOLTING TOP IN 6 FACE IN HIGH TOP AND BLACKED OUT TWICE - FALLING FROM WALK THRU BOLTER APPROX 6' LANDING ON HEAD & RIGHT SIDE - CUT ON TOP OF HEAD - BRUISED & STOVED RT SHOULDER, ELBOW, RT THUMB AND BRUISES ON RT SHOULDER & BACK.
EE WAS WORKING ON 602 SCOOP AND WAS WALKING BEHIND IT AND FELL OVER A ROCK - HURTING LOWER-MIDDLE BACK AREA. AROUND SPAD #2556.
EMPLOYEE WAS GETTING INTO SCOOP AND HIT RIBS ON LEFT SIDE OF SCOOP CANOPY BRUISING HIS RIBS. HE WORKED WED AND THURS AND RIBS WAS BOTHERING HIM AND HE WENT TO DR. EE WAS OFF FRIDAY AND SAT , REPORTED BACK 10-20-03.
EE WAS WALKING AROUNT RT MINER AND SLIPPED ON SLICK BOTTOM AND FELL ON HIS SELF RESCUER - CAUSED INJURY TO HIS BACK WHEN HE LANDED ON IT. EE WENT TO HIS REGULAR DOCTOR FOR SOMETHING FOR PAIN - WAS GOING TO DO AN MRI. REPORTED 8-2-03.
WALKING TOWARDS THE FACE OF #5 ENTRY AND HIS FEET SHOT OUT FROM UNDER HIM AND TWISTED LEFT KNEE EE WENT TO DR POSSIBLE LIGAMENT DAMAGE.
SLIPPED ON SLOPPING BOTTOM- LOST FOOTING FELL WITH LEFT FOOT FOLDED UNDERNEATH INJURED.
STEPPED IN A HOLE CARRYING A CINDER BLOCK. HE TWISTED HIS RT. ANKLE. WENT TO THE DR., HE IS ON CRUTCHES BUT IT IS NOT FRACTURED.
EMPLOYEE CAUGHT FOOT IN BUMPER OF SHUTTLE CAR WHILE GETTING OUT OF CONVEYOR. CAR WAS JACKED UP.
EE WAS HANGING CABLE WHEN HIS FEET SLIPPED OUT FROM UNDER HIM. BOTTOM SLICK. HURT HIS BACK.

WALKING DOWN #5 ENTRY EE SLIPPED ON BOTTOM FELL ON RESUCURER INJURY TO RT HIP & LEG NUMBNESS IN RT LEG & BURNING IN FOOT
FELL GETTING OUT ON MANTRIP ON SLICK ROCK BOTTOM.
EMPLOYEE TRIPPED GETTING OUT OF MANTRIP AND CAUGHT HIMSELF WITH LEFT HAND CAUSING PAIN IN WRIST.
PULLING MINER CABLE ON SLICK BOTTOM AND SLIPEPD AND FELL STRIKING ELBOW AND SHOULDER ON BOTTOM.
EE WAS CUTTING A PIECE OF EXPANDED METAL OFF AT #4 BELT HEAD THAT WAS STICKING OUT IN WALKWAY. A SPLICE IN BELT CAME BY HIT EXPANDED METAL, EXPANDED METAL HIT EE'S RIGHT LEG ABOVE KNEE. EE ST EPPED BACK AND FELL OVER WATER LINE, MAY HAVE TWISTED RIGHT KNEE, IT WAS SWOLLEN A LITTLE.
EE WAS MOVING A REMOTE CONTROL MINER, WHILE WALKING BEHIND IT, EE STOPPED TURNED AROUND TO CHECK SLACK HE WAS PULLING AND STEPPED IN A POTHOLE. LEFT KNEE POPPED. THEN LATER ON IN SHIFT WHILE O N HIS KNEES, LEFT KNEE POPPED SEVERAL MORE TIMES.
EE SLIPPED WHILE GETTING OUT OF SCOOP DECK (LOST HIS BALANCE) TRIED TO CATCH HIMSELF WITH HIS LEFT HAND, EE LEFT HAND HIT GROUND FIRST THUMB OR HAND SPRAINED BACKWARDS CAUSING PAIN.
EE SLIPPED & FELL WHILE CARRYING A BAG OF ROCK DUST. EE SAID WHEN HE FELL HE LANDED ON HIS SELF CONTAINED SELF-RESCUER THAT IS ON HIS BELT.
