THE INDIVIDUAL WAS PULLING ON THE MINER CABLE WHEN HIS FEET SLIPPED ON THE WET PAVEMENT CAUSING HIM TO FEEL PAIN IN HIS LOWER BACK. THE INDIVIDUAL DID NOT NOTIFY CO PERSONNEL THAT HE WAS SEEIN G CHIROPRACTOR UNTIL 10-27-94.
