Showing all 31Employee was standing on the rear wheel wall of a Mac 3D assisting a mechanic lineup motor bolts. As he exited from this position his right knee twisted causing him to fall onto the shop floor, striking his right elbow and head.
Employee pulled a piece of draw rock. When he started to clear the rock from the roadway he twisted, then stumbled and fell. He experienced a sharp pain in his back and down his left leg and knee.
The employee stated that he was at the #6 belt drive when he slipped and fell hitting his back on a small pebble which resulted in his lower back hurting.
EE was greasing feeder. When he walked around the fire boxes and the section tailpiece, he tripped over the hydraulic drill and hoses.
The employee was taking down a ventilation curtain when he stepped on a rock, which resulted in the employee falling and injuring his right hip and back.
EE was getting out of his coal truck to clean his windows after crossing the truck wash. He said his foot slipped off the bottom truck step landing hard on the ground. He said he fell backwards landing on his right hip. He had severe pain in that hip. He was taken to Harlan ARH emergency room, x-rayed and diagnosed with a large contusion, no broken bones.
Date of original injury was 2/29/12. The employee returned to work on his next scheduled shift. The employee continued to work until 9/17/12 when he stated that he could not continue. The employee was examined on 9/18/12 and taken off work. From the medical reports it has been determined that his back injury is related to the original injury that occurred.
Employee was removing a lid from the miner when he lost his balance. He fell with the lid and the lid struck the miner frame with the employees right index finger between the two. Employee was taken to Harlan ARH Hospital then sent to Holstien Valley Hospital. He seen the Doctor again on Monday, Feb 13, 2012. He worked Monday night on his regular shift.
Employee was installing a strap with a roof bolter. 1 end was bolted to the top. As he was trying to push it around to get it straight, his footing slipped and he fell against the strap resulting in a cut to his wrist. Cut was between gloves and keblar sleeve. He was taken to Whitesburg ARH and received 5 stitches. He was released to full duty and returned to the mine.
Employee was bolting top when a rock fell striking his canopy which caused him to run. While running, he tripped and fell striking his head and the RT side of his body. He was knocked out for 30 seconds. He was complaining of pain to his RT side, chest and left knee. Injuries were due to fall and not from rock falling. Was transported to Harlan A.R.H.
EE was hooking up duster to scoop bucket, the hoses were tangled as he was stretching the hoses out the duster fell off of the scoop bucket jerking him down.
The EE was operating a CM The EE was attempting to move the miner cable back when he stepped into a hole causing his left knee to pop which caused him to also strain his back.
Employee was helping a truck driver clean out tailgate on a truck, when he slipped on ice letting the tailgate close on his left hand. Laceration and fractures to ring finger 6 Stitches.
Employee was helping roof bolter move around to spot a bolt, when he stepped into a hole straining his lower back.
Employee was pulling roof bolts off the rear of the roof bolting machine and twisted his back. No medical attention received at this time by employee. Several days later employee calls and reports broken bone in the left foot which occurred when he stepped off the rear of the machine to the ground. Report is being sent to show injury to left foot.
Employee was working on top of an overcast in the no. 5 entry of 001 section. The employee started to step down or jump down when his light pouch got caught on overcast causing him to twist before reaching the ground, injuring his right leg.
Employee went to step up on equipment when he pulled or twisted his left knee. Employee had knee examined, did not miss any work. Knee getting worse. Had surgery on knee on 11/3/08. Back at work on 11/4/08 on restricted duty.
Employee was walking through water and tripped over a rock injuring his back.

EMPLOYEE WAS INJURED WHEN WALKING IN #4 BELT ENTRY WHEN HE STEPPED IN A RUT, TWISTING HIS RIGHT ANKLE.
THE INJURED WAS POSITIONED ON A LADDER SETTING A ROOF SUPPORT JACK. (MINING HEIGHT APPROXIMATELY 12'). HE FINSIHED SETTING THE SUPPORT AND WAS DESCENDING THE LADDER WHEN HIS FOOT SLIPPED ON THE 3RD RUNG. HE LANDED ON THE MINE FLOOR AND TWISTED HIS KNEE.
EMPLOYEE WAS LIFTING BELT LINE WITH CUMMALONG. THE CHAIN BROKE, BELT LINE FELL AND KNOCKED HIM TO GROUND STRIKING SHOULDER ON A PIECE OF STEEL. APPROX 30 MINUTES LATER, THER WERE RAISING BELT AGAIN WHEN THE BOLT HEAD BROKE STRIKING HIM IN LEFT SHOULDER. HE FINISHED SHIFT, WENT TO ER. WORKED ON 7/6/05 THEN A MESSAGE FROM HOSPITAL THAT HE CRACKED A BONE IN SHOULDER
INJ WAS CARRYING 2 5 GALLON CANS OF HYDRAULIC OIL WHEN HE SLIPPED ON A SHUTTLE CAR CABLE & TWISTED HIS LEFT KNEE. CONTINUED TO WORK HIS REGULAR JOB AND DID NOT MISS ANY TIME UNTIL HE HAD ARTHROSCOPIC SURGERY TO REPAIR CARTILLAGEN 2/2005
EE was carrying bolts from one end of walk thru drill to the other. The ee stumbled on a rock and fell against the drill hitting left arm.
EMPLOYEE WAS WATCHING THE ROOF BOLT CABLE FOR THE OPERATOR WHEN THE CABLE JERKED AND PULLED THE EMPLOYEE'S FEET OUT FROM UNDER HIM. THE EMPLOYEE FELL AND HIT HIS SHOULDER.
THE INJURED WAS USING A LADDER TO HANG BELT CHAINS. THE LADDER KICKED OUT FROM UNDER HIM CAUSING HIM TO FALL BACKWARDS. HIS HEAD STRUCK THE MINE FLOOR CAUSING PAIN IN HIS NECK.
HE FELL IN A SCOOP BUCKET.
EE WAS HANGING LINE CURTAIN USING 8' STEP LADDER. LADDER WAS PLACED ON GOB & WHEN INJURED STARTED TO CLIMB, LADDER STARTED TO TILT. INJURED JUMPED OFF LADDER BACKWARDS AND HYPEREXTENDED HIS KN EE. FINISHED SHIFT & WENT TO E.R. LATER THURSDAY.
EE WAS LOADING STRAPS ON ROOF BOLTER HE FELL OFFCATCHING HIS FINGER IN THE STRAPS CUTTING IT. IT REQUIRED 3 STITCHES
3/23/00 EE WAS RUNNING A ROOF BOLTER & TRIPPED OVR THE CABLE FALLING INTO THE BUMPER OF THE ROOF BOLTER, HURTING HIS RIBS. HE DID NOT GO TO THE DR. 4/6/00 EE WAS LOADING A BELT STRUCTURE INTO SCOOP & FELT A POP IN HIS RIBS. EE THEN WENT TO THE DR.
