The employee stated while he was shoveling the 4 West belt he lost his footing and started to fall, but reached out with his right hand to catch himself. He grabbed a piece of top structure at the wing roller and caused a laceration to the tip of his right middle that required sutures. The incident is a medical reportable injury.

Showing all 13EE stated that his feet got tangled up in a water hose as he walking toward the #1 press to wash it. Fell to the floor straining his left hand and wrist. 10/9/15 incident became a reportable injury because ee sought medical attention and was diagnosed with a possible hairline fracture and prescribed a wrist brace. He returned to work at his normal job his next scheduled shift.
The employee stated that he was exiting the 507 scoop and as he pushed himself out with his arms, his right foot slipped out and he fell towards his right leg. This caused his right knee to bend inwards toward his left leg. The incident became a medical reportable on 9/17/2015 due to the employee receiving physical therapy.
The employee stated he twisted his right knee while he was dismounting a 988 loader. The employee initially sought medical attention but did not receive any medical treatment and returned to work. On 1/22/15 the incident became reportable due to follow-up medical care. This is being reported as a medical incident. *On 5/13/15 the EE began missing work.
The employee stated that during a belt move he crossed the belt (locked and tagged out) to get a chain hoist and when he crossed back over he slipped and fell striking his right knee on the water manifold. The employee did not return to work on the 13th. This is being reported as a lost time injury.
EE was unloading bag dust from one scoop to another and his right knee twisted causing pain. Later in the shift he exited the scoop. While walking around the batteries, his foot got stuck between the side of the batteries and a rock that caused his knee to turn and pop. He sought medical attention and was diagnosed with a sprain/strain.
EE stated that the 1 North section belt head had went off on gob and he stepped up on the side of the flow-thru to see inside of the rock box. As he was stepping down he grabbed a cinder block that was sitting on the flow-thru that gave way and caused him to loose his balance and fall against the mine floor and onto his lower R leg. Diagnosed w/fractured leg, on restricted duty.
The employee stated he was walking to get on his shuttle car when he stepped in a hole and twisted his left knee causing a strain/sprain. The employee continued to work until 2/24/14. The employee began missing work on 2/25/14 while under doctors care.
The employee stated he was pulling a hand dolly across the track at the Spice Lick Shaft Bottom and tripped and fell. While falling, he tried to catch himself and fell onto his right shoulder causing a strain. This incident became a medical reportable on 2-14-14 because the employee was seen for a follow-up medical visit that resulted in physical therapy. There were no lost days.

ACCORDING TO THE EMPLOYEE, HE WAS WALKING TRAIN AS THE BRAKEMAN WAS RELEASING HAND BRAKES. TO DO THIS, YOU MUST CLIMB UP ONTO THE SILL STEP IN ORDER TO REACH THE HANDBRAKE. HE HAD WALKED THE CUT OF CARS & WAS 5 CARS FROM THE LOCOMOTIVE. HE WAS ON THE CAR & HIS FOOT SLIPPED OFF, HE HELD ONTO THE HANDHOLD TO CATCH HIMSELF, STRAINING HIS BACK.
EE SLIPPED ON ICE & FELL. HE TRIED TO CATCH HIMSEL;F AND CUT HISRT. HAND ON A SHARP OBJECT.
