EE had closed the gate at the mine entrance and was walking back to his truck, he slipped on the metal grate injuring his shoulder. Due to conflicting reports of injury location a medical review was conducted to determine the validity. On 8-24-11 the report indicated the injury was related to the accident. He started missing work on 8-4-11 and surgery was performed on 8-5-11.

Showing all 12Member was walking along #14 belt and bent down to cross under a water line, slipped and fell on an old piece of conveyor belt, causing a laceration to his knee. He went to Williamson Memorial Hospital ER where he received stitches and a full release to return to work.
While shoveling slope belt employee lost his footing and twisted his left knee. He continued to work until 07/27/2010, when he had surgery to repair a torn meniscus and a tear behind the knee.
Cleaned #1 magnet at glory hole, turning to go down steps, heard noise, turned to look, missed top step, fell down stairs, injured right knee.
Crew had completed belt move. Was hanging tilt switch, stepped down from tailpiece, stepped on a rock and fell to the ground and sprained his ankle. Worked next two days, went to doctor on 8/6/07, took off work until 8/13/07 and RTW at full duty.
While bolting in the #3 entry on 011-0 MMU, EE states his foot got caught between loose draw rock and caused him to twist his left knee.

MEMBER WAS CROSSING BELT CROSS-OVER AND SLIPPED AND FELL. MEMBER CONTINUED TO WORK UNTIL JULY 11, 2005. *NOTE: MEMBER FIRST DAY OFF 7-11-05.
EMPLOYEE WAS 1 BRK INBY THE FEEDER. HE GOT OUT OF HIS CAR TO HELP HANG A MINER CABLE. HE SLIPPED ON A ROCK AND TWISTED HIS RIGHT KNEE. HE WENT TO THE ER, WAS EXAMINED, AND GIVEN A RTW RELEASE. EE CONTINUED TO WORK, PAIN IN HIS KNEE INCREASED AND HE SAW THE DR IN LEXINGTON. HE HAD SURGERY ON 2-14-05 FOR A MENISCUS TEAR, AND RECOVERY TIME IS EXPECTED TO BE ABOUT 2 MONTHS.
EE WAS SETTING A PUMP ON BELT #15 AT BREAK #16, HE TRIPPED OVER A RAIL. AS HE FELL HE TRIED TO BREAK HIS FALL, INJURING HIS LEFT ARM, LEFT HAND/WRIST, AND HIS BACK.
EMPLOYEE WAS OPERATING A ROOF BOLTER WHEN HE LOST HIS FOOTING. HE PUT HIS RIGHT HAND ON THE DRILL POD TO PREVENT HIMSELF FROM FALLING. A PIECE OF DRILL STEEL FELL OUT THE BOLT HOLE AND STRUC K HIS RIGHT RING FINGR. CAUSING A LACERATION REQUIRING STITCHES.
AFTER EXPERIENCING A POWER OUTAGE, EE WAS WALKING TO THE FACE OF THE #2 ENTRY TO CHECK ON THE CONTINUOUS MINER. AS EE WAS TRAVELING THRU THE LAST OPEN BREAK, HE STEPPED ON A LUMP OF COAL. HE S TATED THAT HIS ANKLE CREELED TO THE OUTSIDE CAUSING HIM TO FALL TO THE MINE FLOOR. EE SUSTAINED A FRACTURE TO THE FIBULA AS WELL AS A HAIRLINE FRACTURE TO THE ANKLE BONE.
