The employee was doing a ground level repair to a water valve. While exiting the repair area, they attempted to step over a water pipe and a connected hose. Their feet did not clear this structure, and they lost balance and fell onto a concrete slab causing injury to their right shoulder. No safety rule violations were attributed to this incident.

Customer was filling water tank on pick up truck. Stepped up on rear bumper of truck and slipped, falling backwards landing on berm, fractured ankle. Bumper was 27" above the ground.
