EE lost balance while removing metal sheeting from inside 32B drive house, falling approx. 16' to the concrete floor below.
EE was dismounting off of 49-K screen when foot slipped causing a pain in left knee. EE continued to work without restrictions. Due to Covid-19, surgery was postponed until 7-8-2021
EE slipped and fell while walking up steps in prep plant and twisted knee. EE continued to work until an MRI could be scheduled. MRI on 5/4/20 revealed a meniscus tear and other damage and was taken off work until 5/11/20. EE returned to work full duty and is currently awaiting a surgery date. However, elective surgeries are not being conducted at this time due to COVID-19.
Employee hurt left wrist/hand when the employee slipped and fell while carrying a sample bag of coal down a flight of steps at the Analyzer Building.
EE was installing floor drain boxes into the ceiling of the 8th floor of the preparation plant. Said EE fell off the scaffolding falling 5' hitting left shoulder.

Employee slipped on ice covered roadway while working outside at the loading facility. Employee continued to work until May 16, 2011 when his doctor took him off awaiting knee surgery.
Person pulled back top of 54 sieve chute to allow sieve to clear when rotated. Person was standing on platform and tripped on chute and fell forward and scraped his shins on the top of chute causing injury. Person received medical treatment on 10/14/10 and was released.
Injured person was changing pump impeller with another person. Injured had pipe wrench on impeller trying to break it loose. The pipe wrench slipped, causing person to fall backwards on his left leg and back. Person finished shift and began to miss work after MRI. Persons first day off was 4/13/10.
Injured person was inspecting center well section that had been removed from thickner and was on the ground. Person slipped on metal and lost footing, he put his hand out to catch himself and cut his left index finger on sharp piece of metal as he fell.

EE was walking up grade from propane tank to company truck. As he was walking up hill he felt a sharp pain in right ankle and leg. EE went to chiropractor on 5/15/06. *Person had to have surgery on right ankle and began to miss time on 2/14/07.*
Person was carrying bag of rock dust on shoulder to use for dusting on beltline. When he walked past starter box he stepped on rubber mat and slipped and fell causing injury to left knee. Person now has to have surgery on knee, first day off is 9/21/06.
EMPLOYEE WAS UNLOADING SUPPLIES FROM SCOOP BUCKET & STEPPED OUT OF THE BUCKET, TWISTING HIS RIGHT ANKLE CAUSING A SPRAIN.
Person was helping to get belt discharge pulley through 6' door in stopping. The pulley moved unexpectedly and person lost balance and fell backwards. Person put his left land out to catch himself and this caused injury to left shoulder. Person is off for surgery to left shoulder as of 9/21/05.

Injured was carrying a post with another employee. The employee slipped forward causing post to go into rib. The injured had his finger between post and rib causing a laceration of the right index finger.

EE was installing light ballast for support during longwall set-up. EE was moving from #22 to #23 support and lost balance. He grabbed hose that was not attached and fell forward, striking his right shoulder against side flap. EE went to doctor on 4-15-05 & got cortisone shot. SURGERY REQUIRED - WILL BEGIN TO MISS WORK AS OF 8/25/05.
EE WAS WALKING ALONG TRACK TO GET TO MANTRIP AT END OF SHIFT. EE STEPPED IN A HOLE BESIDE THE RAIL AND LOST HIS BALANCE. EE FELL FORWARD, CATCHING HIMSELF WITH HIS LEFT ARM, CASING PAIN IN SHOULDER. EE WENT TO DOCTOR ON 3/28/05 AND IS NOW OFF WORK FOR SURGERY TO SHOULDER.
Employee was walking through a 4'X 6' mandoor at the submains belt starter (3 break). He slipped and fell causing injury to his ankle.

PERSON WAS STANDING ON WALKWAY OF BOLTER PUTTING UP THE HARDWARE TO CONNECT SLINGS THAT HAD BEEN INSTALLED FOR ROOF SUPPORT. GRADE IN AREA IS STEEP & PERSONS FOOT SLIPPED OUT & HE FELL TO THE METAL WALKWAY ON HIS BUTTOCKS. EE WENT TO A CHIROPRACTOR.

INJURED PERSON WAS LEAVING THE LONGWALL FACE TO EAT LUNCH. WHEN HE WENT AROUND THE HEADGATE DRIVE UNIT HE TRIPPED OVER A 4 FT ROOF BOLT AND FELL. HIS RIGHT HAND CAME DOWN ON SOE SHARP ROCKS CA USING LACERATION TO HEEL OF RIGHT HAND.
Person was walking up ramp at belt overcast and his feet slipped out from under him and he fell on both knees. person has now gone to dr and has been diagnosised with a torn menisus. he is now off work first day off 5-6-04.

INITIAL DIAGNOSIS WAS A SPRAIN BUT MRI SHOWED TORN MENISCUS. SURGERY ON 08/17/2004. DISCOVERED THAT WE HAD FAILED TO SUBMIT A 7000 FORM WHEN EE RETURNED TO WORK ON 10/25/2004.
PERSON WAS WALKING BESIDE MANTRIP WHEN HE SLIPPED ON PIECE OF FLAT ROCK AND FELL. WHEN PERSON FELL HE TWISTED HIS LEFT KNEE AND FELL AGAINST OPEN DOOR ON MANTRIP, HITTING HIS UPPER LEFT ARM.

INJURED TWISTED HIS KNEE WHILE CROSSING UNDER U98 BELT. A SMALL AMOUNT OF SPILLAGE WAS PRESENT. HE WENT TO A CHIROPRACTOR FOR TREATMENT ON 9/10 - HE WAS TREATED AND RELEASED WITHOUT RESTRICTIO NS. HE NOTIFIED THE COMPANY WHEN HE RETURNED FROM SCHEDULED VACATION ON 9/15.
EE WAS COMING DOWN A SET OF STEPS AND CAUGHT THE HEEL OF HIS BOOT CAUSING HIM TO SLIP & FALL. HE HIT THE GROUND AND STRAINED HIS RT. FOREARM TRYING TO BREAK HIS FALL.
INJURED WAS PULLING ON MULE CABLE WITH OTHERS TO REANCHOR IT. HIS FOOT STUCK IN THE MUD CAUSING HIS RIGHT KNEE TO TWIST.
