Showing all 10EE had been sick all morning and was vomiting, he had decided to go home if he vomited one more time. He pulled to the side of the road to vomit and when he leaned over the handrail, he apparently blacked out and fell 11 feet to the ground.
He was working on a dust collector. When he stepped onto the deck, his foot slipped, and knee made a popping sound. It was raining at the time, so deck was wet.
Fell backwards out of back of service truck while stepping to bumper.

Employee was coming down from off a machine when his foot slipped and he twisted his left knee.
Employee was climbing down off of a Caterpillar 992G and as he was climbing down he grabbed hold of a hose as a hand hold instead of the ladder. In doing so, he tore, possibly fractured, left shoulder.
Employee was walking down slope on valley fill when he slipped and fell, landing on his arms and shoulder. EE did not miss work until 11-11-04 when he had to get surgery.
