INJURED WS CROSSING UNDER BELT WHEN HIS FOOT SLIPPED ON A WET FLOOR & COAL FINES. INJURED LOST HIS BALANCE AND HIT HIS NOSE AND UPPER LIP ON BELT GUARDING. THE INJURY REQUIRED STITCHES TO HI S NOSE AND UPPER LIP.

THE INJURED WAS STANDING ON THE BACK OF THE ROOF BOLTER PUTTING SITES LINE ON ROOF, WHEN HE STEPPED DOWN. THE MINE FLOOR WAS UNLEVEL AND THE RIGHT FOOT WENT SIDEWAYS SPRAINING HIS RIGHT ANKLE.
