Icy conditions on deck of crane. Operator was leaving cab and slipped on deck of crane and fell to same level. The operator grabbed the cab door on way down causing minor strain to right shoulder. The operator's hard hat came off, EE's butt hit 1st causing bruising to right humerus, and back of head struck platform resulting in a contusion and hematoma.

EE finished the shift and sought medical treatment and was given a full release but was diagnosed with a rib fracture. EE returned to work on EE's next scheduled shift and has not missed any work.

Note this was his second day at the mine, his first day was finishing his training.
Employee stated he tripped over cable falling into shuttle car striking ribs. Employee finished his shift. He sought medical treatment on 1/27/14 and x-rays were negative. Employee continued to work until he sought medical treatment for continued pain 2/20/2014 and CT indicated a fracture of 9L rib. Employee continues work at full capicity and has not missed any work.
Floor was wet and muddy, while carrying roof bolts plates to supply bolter. Employee slipped and smashed finger between roof bolt plates and bolter. This resulted in a 3/4" laceration to left middle finger that required four sutures.
