Bent a bolt on the dust box and slipped off the bolt & hit his right eye on the edge of the pinner. He was putting up 2nd roll of bolts. There were some rocks on his side.

Climbing up ladder to access wier plates to change out wier plates. Ladder was tied off at top, but employee's boots were slippery due to conditions. Ladder shifted slightly and boots slipped causing him to fall against the inside edge of a wier plate at same elevation.

EE was standing on a ladder, one foot on the ladder and the other foot in the generator room. In an attempt to pull the breaker, he slipped and fell approximately 5 feet, injuring his back and hip.

EE was assisting in replacing LCM Cat wear strips. The miner was elevated on cribs for the repairs to an approximate height of 5.5 feet. Repairs were completed. EE climbed on top of the miner to take down the work area lights. To get off the top of the miner, EE got on his hands and knees and jumped off backwards. Upon landing he twisted the left ankle.

EE tripped on a rock measuring 8"l x 6" w X 14" to 6" thickness and struck his left knee on a cable hanger on the operator side of the miner. He was transported to Clinch Valley Medical Center for testing and was released.

EE & co-worker were setting beams & jacks. Beams were on a flat car approximately 20" from mine floor. Beam was raised & chained. EE moved to the other end of beam to pick up & set jack. First jack & beam became dislodged. To get out of way, EE proceeded to end of flat car & fell head first, contacting skull with sharp rock.

A PIECE OF BELT WAS BEING PULLED IN 3 BREAKS OUT BY THE TAILPIECE HEN THE BELT CAME OUT OF THE ROLLAND STRIKING EMPLOYEE. HIS FOOT WAS AGAINST THE RAIL AND HIS BODY WAS ARCED BACKWARDS. HE HAD A FRACTURE TO THE TIBIA DISTAL TO THE KNEE. HE DID NOT NEED SURGERY OR A CAST AND HE RETURNED TO WORK HIS NEXT SHIFT.
