Showing all 8Employee was pulling a rope across the spillway between pond #1 and pond #2 when ee slipped twisting left knee.
Employee was working on a loft step ladder. EE went up the ladder to grind a bevel. EE got to the third step and the extension cord that EE had for the grinder was short. As EE turned to pull on the extension cord to get more slack, feet slipped on ladder rung causing EE to hit side on top of ladder.
Member was replacing the cutting edge on a blade for a bulldozer using a 1/2" impact gun. As he was doing this he stumbled striking his back on the tracks of the dozer. Member brought in documentation from his doctor on 5/27/2010
EMPLOYEE WAS WALKING OUT ON THE THICKNER AND AS HE WALKED HIS FOOT SLIPPED THROUGH A HOLE WHICH WAS CAUSED BY A DEFECTIVE WELD. HIS LEFT KNEE HIT THE GRATING CAUSING A LACERATION REQUIRING STITCHES.
WALKING BEHIND BELT LINE, TRACING A CUTTING TORCH LINE. WATER WAS APPROXIMATELY ANKLE DEEP SO A SUMP HOLE COULD NOT BE SEEN. THE GRATING HAD BEEN REMOVED AND NOT PUT BACK. EMPLOYEE FELL AND CUT RIGHT HAND.
EE WAS DISMOUNTING A KATMASU ENDLOADER TO CLEAN COAL FROM A COAL PAD WHEN HE SLIPPED AND FELL FROM THE LOADER LOADING ON HIS BACK.
