Employee fell onto employee's proximity pad at the right hip resulting in contusion and bruising to the right hip. Employee was seen the same day and given a full release with no restrictions. During a follow up appointment the employee was placed on restrictions preventative in nature.
An accident occured and a miner tried to catch another miner who was falling. The miner has been on leave since the accident. The miner reported shoulder pain the following day and was diagnosed with a shoulder sprain several weeks later.

Reinjured an existing injury to EE's left foot fracturing the fourth metatarsal. Injury was due to excess body weight on the employee. Employee stumbled catching self and applying all weight to the one foot.

EMPLOYEE STATES HE WAS INVESTIGATING WHAT WOULD BE NEEDED TO PULL OUT A PLOW TRUCK THAT WAS STUCK IN THE SNOW. HE WAS WORKING HIS WAY DOWN THE SIDE OF THE VEHICLE, HOLDING ON TO THE SIDES, WHEN BOTH HIS FEET SLIPPED OUT FROM UNDER HIM. HE FELL TO THE GROUND, LANDING ON HIS RADIO WITH HIS LEFT LEG, MID-THIGH, RESULTING IN A FEMUR FRACTURE.
Employee picked up a bottle of oxygen off the welder trailer, slipped and fell and hit his back against the hitch on the trailer.
Employee was kneeling and stood up. Stepped on harness lanyard. Fell forward hitting left knee.

Employee states he was welding on a refuse bin and slipped in the mud when relocating the bin. The employee jammed his right thumb as he was attempting to stop his fall, hyper extending his thumb. Employee sought medical treatment and worked 5 days on modified duty.

Tripped on shield toe and fell cutting his thumb on side of shield toe.

Employee was simply walking down the steps. When she stepped down her ankle twisted. She didn't slip, trip or come across any hazard. She went to the ER where she was told she has torn ligaments in her ankle and must see an orthopedic surgeon before being released back to work.
When stepping from one shield to the next, the employee slipped, striking either the panline or relay bar, resulting in a dislocated shoulder.

Employee states he was walking between the rib and the rockdusting machine. He stepped on a lump of coal, causing him to fall to the ground, catching himself with his hands, resulting in a fractured bone in the left hand. Reportable medical treatment was not provided until 5/10/2005.
Employee states he was helping hang vent tube, he reached up to grab roof screen to maintain 3 point contact and roof screen pulled out of roof and he fell to ground hitting his back. This did not turn to a medical reportable until 04/06/05, with treatment of a physical therapist and employee did not report therapy results to mine until 04/10/05.
Ee states while climbing on a scoop to drop tubes, his foot slipped & he fell catching himself w/his left arm. This caused ee to have pain although he continued to work. On 7/9/04 he received an MRI & on 7/14/04 the results were provided to ee & co. to be that he had a torn rotator cuff requiring surgery. Surgery is scheduled for 7/21/04.

STANDING ON DECK BUILT AROUND DRILL TO SECURE A CABLE OVERHEAD. DECK BUILT OF WOOD AND WAS SLICK DUE TO WATER. DECK BOARD MOVE AS IT WAS NOT PROPERLY SECURED OR BECAME UNSECURED. EE FELL APPRO X 2' TO THE GROUND, STRIKING UPPER TORSO ON THE DRILL (CONTROL PANEL GUARD PLATE).
HIGH AIR VELOCITY ONCE DOOR WAS OPENED.
EE STATES WHILE WALKING IN BELTLINE, THE EDGE OF THE WATERWAY GAVE OUT CAUSING HIM TO FALL RESULTING IN A SOFT TISSUE INJURY TO HIS RIGHT ANKLE. MEDICAL TREATMENT WAS NOT SOUGHT UNTIL 7/15/00.
EE STATES HE WAS CLIMBING DOWN FROM A LADDER, LOST HIS FOOTING AND FELL, LACERATING HIS RIGHT ARM WHICH REQUIRED SUTURES.
