The injured was tightening the brake adjustment on the slope hoist. The wrench he was using slipped off the nut and the injured fell back and struck his head. He received a laceration that required two sutures. This is being reported as a medical treatment accident.

The wooden ladder lost balance and kicked out from under him. He fell striking his right knee/shin on the motor and striking his left shoulder on the breather exhaust.
When getting out of mantrip employee tripped and caught his right finger in tensor when he braced for the fall.

While walking, employee slipped on a cable and fell at the stationary loader. When he fell, he caught himself with his right hand causing an over extension of his right wrist and a simple fracture to the right hand.
Employee was stepping onto a motor and jammed his head into the roof causing him to fall backwards landing hard on his back. Neck and lower back pain.
The injured stepped off a supply motor and twisted his ankle. the injured worked till 08/08/2014 the day of his surgery. Due to poor communication we did not realize that he went off work for his surgery. that is why we are late reporting this incident.
Employee slipped and lost his balance falling backwards and landing on his self rescuer as well as a bit bucket. The injured stated the bit bucket struck his lower back. Injured had complaints of back/tailbone pain and tingling down his right leg.

The injured was dragging a rockdust hose when he slipped on a wet walkway. As he fell he reached out with his right hand to brace himself. When he hit the ground his right hand contacted an old roof strap that was laying in the c.c. causing a laceration to the base of his right hand requiring 5 sutures.

Employee stepped in a hole with slick bottom. He lost his balance and when he fell heard a pop in his right knee. He felt some immediate pain.
While installing 10" pipe, employee was pushing pipe together and his foot slipped on a wet surfece causing him to twist his right knee.

EE attempted to stop his fall by clasping on to the panline handrail. A strain to his right bicep resulted from the clasping. The strain was later diagnosed a muscle tear to the bicep requiring surgery.
The employee was greasing the belt drive at the preparation plant clean coal silo and tripped on an air hose. The employee fell, lacerating the end of his left middle and ring finger. Sutures were not required, but the physician was concerned about infection and instructed days away from work.
The track crew was preparing to lay track on a continuous mining section. The injured employee was handling the track ties when he slipped and fell to the ground. He tried to brace himself with his right hand, causing a fracture to his right hand.
On 3/16/12 the EE slipped while climbing from a scoop, striking his right chest area on the scoop tire. The EE continued to work and requested a follow-up exam. The exam. and x-ray on 3/26/12 showed a fracture to his right rib. The EE missed no work and continued performing regular duties.

The EE's fist day off was 1/16/12 when surgery was performed.
On 8/26/11 EE turned in an incident form stating that he was pulling a 1" hose and slipped and fell.On 9/2/11 EE turned in incident form stating that he had sneezed and felt something pop in the area he struck when he fell. He went to a Dr.,bone scan was performed and the results of 3 fractured ribs was reported on 9/15/11.EE cont'd. to work at his regular duties during this time.
