Incident occurred at night with light rain condition. Employee's foot slipped while climbing wheeled loader ladder. Employee strained right shoulder when he started to fall. Employee worked with some pain from 3/23/10 until 4/12/10. On 4/12/10 he was seen by medical service, diagnosis rotator cuff strain. on light duty 4/12/10.

On the day of the injury, the employee was at the site early to start equipment up for the start of the shift. He had just started the D5 Dozer and was attempting to egress it, when his right foot slipped off of the equipment provided step. His left foot made contact with the ground and twisted, as a result, a bone in his left ankle was broken. (sprained not broken per MM)

Clear, dark conditions: got loaded with coal and had a problem with the tarp. The tarp is manually operated and the driver climbed up to untangle the tarp rope. When the tarp untangled, driver lost his balance and fell against the trailer in the coal. No initial medical treatment but placed on restricted duty on 1/7/08.

The employee reported that he had been working on a silt pan. He exited the pan by climbing over the side. Upon exiting he felt a strain in his back. On 11/21/05 employee visited a physician who believed he had a pinched cyatic nerve. It wasn't reported until 11/21/05

EE OPENED THE DOOR TO THE OFFICE TRAILER AND STEPPED ONTO THE PLATFORM. HE STARTED TO CLOSE DOOR AND SLIPPED ON ICE AND FELL DOWN THE STAIRS, FEET FIRST. THE EE HIT HIS HEAD, RIGHT ELBOW AND R IGHT RIBS ON THE BACK ON THE WOODEN STAIRS.
