Employee was standing on a ladder to change out bread crumb in the 4th East Mains section. The ladder slipped on the mine floor and the employee fell to the ground and hit right elbow.

Employee was carrying a ladder down the stairs of the overcast on #7A beltline located at crosscut 46 in the 3rd East Mains, when employee lost balance on the 3rd from the bottom step and sprained left ankle.

Employee was standing on the roof bolter attempting to put a metal roof mat on top of the canopy and as EE stepped from one side of the bolter to the other side, EE lost footing and fell and hit head on the ATRS scissor hinge area, resulting in a laceration which required 4 stitches to EE's right eye brow.

Employee was walking down the longwall face from the headgate to the tailgate, foot slipped on the wet surface of the mine floor and as employee fell, struck left knee on the toe of shield #80. 5 stitches were required to close the laceration on left knee. EE was released to return to work but had a followup visit on 2/25/19 and is currently off work pending an MRI.

Employee was walking down the slope along #4 belt conducting the pre-shift examination when EE's foot slipped and EE fell to the ground. EE reported the incident. EE went to the doctor on 2/27/19 and had a MRI on 3/6/19. EE had a followup visit on 3/14/19 and was told EE had torn meniscus in the right knee. Lost time injury as of 3/14/19.

A contract miner was standing on the 3rd rung of a 6' ladder, as the miner leaned to one side and attempted to hang a belt chain from a roof bolt, the ladder tipped over and the miner hit head on the ladder as miner fell to the ground. 3 staples were required to close the wound. The miner was released to return to work on next shift.

Employee was walking down a set of wooden stairs at the overcast @ XC 29 in 5th South when the second from the top plank gave away and employee fell to the ground injuring left knee, right shoulder and neck.

Employee was operating an Isuzu truck at XC 27 in 3rd East Mains when the alternator starting squealing. The operator stopped the truck and got out of the truck. As employee was walking towards the front of the truck to open the hood, employee's foot slipped on the wet roadway and employee strained lower back.

Employee was operating the Wagner 2DS Can setter #24-43. Pulled into cross cut #18 in the #2 entry of the 3rd East Mains to park the machine. As employee went to exit the operator compartment, when placing right foot on the ground, right knee popped and buckled underneath the employee.

Employee was kneeling on the deck lid of a belt transfer chute that was being installed. EE was attempting to remove a pin that was stuck in the adjustable legs of the chute. As EE jerked real hard on the pin, the pin came out of the hole and the employee lost balance and fell to the ground resulting in a fracture of right arm and a hairline fracture to pelvis.

EE was attempting to put air into the left front tire of the Genie forklift when the inside wall of the tire blew out. EE stumbled backwards and fell over a stack of crib blocks thus injuring lower back. X-ray were negative, EE was given a prescription for back spasm.

The EP was carrying a Kennedy Jack on shoulder, stepped into a muddy rut and twisted left knee. An initial report was filed but no medical action was taken until 7-11-16 when the EP complained that their knee was bothering them. EP was taken to the Emery Med. Clinic where they were given a knee brace and a prescription for a topical ointment resulting in a reportable.

The person was changing a coupler on a 8" Aqua Mine pipe. EE was standing on a 6' ladder. When EE stepped off the ladder, EE stepped on the rung on the side instead of the middle causing the ladder to tilt. EE fell on back on the right side. Resulting in 3 fractured ribs.

The employee was unwinding cable slack from the section's face fan. While untangling a spot in the cable he stepped backwards and his right foot was placed on an uneven spot on the mine floor causing his right ankle to twist/roll. He was taken to the Emergency Room where X-Rays were taken. It was diagnosed that he sustained a fractured ankle, Fibula bone.
