Employee was strapping down a load on a supply car while standing on the car at the Battle Run yard, slipped on ice, and fell to the ground. EE worked EE's normal job until going off for surgery on 04/16/2026.
We are a water treatment company. This employee is not an employee of Tunnel Ridge. They were getting into position to take a picture for a service report. They stepped on some loose gravel/dirt. Their feet slipped out from under them. They landed on a large rock on their lower right back.
Employee exited a supply locomotive and twisted EE's knee. EE continued working EE's normal job through a series of follow up appointments & physical therapy until being removed from duty on 8/7/25 to have ligament repair surgery.
Employee was walking on the concrete floor near the tail of the North Mains #2 conveyor entry and slipped on wet rock dust.
Employee was part of a crew working to unload and store longwall pan line underground. EE stepped in a hole and fell, striking the left side of their head on a piece of pan line, lacerating EE's left ear.
Employee was exiting the cab of the CAT 980M end loader located at the Battle Run supply yard. They missed a step while climbing down the ladder and fell backwards, bumping their head on the ground.
Employee was carrying a hydraulic hose down the longwall face and slipped on gob, straining right knee. EE worked EE's regular schedule through 06/05/2024. EE was initially seen at a treatment facility on 06/06/2024 and put off work at follow up appointment on 06/11/2024
Employee rolled right ankle while dragging a cable. EE rolled it again later in the shift while fire bossing.
Employee was walking alongside a scoop car, tripped on a rock, and fell, jamming EE's left wrist on the mine floor. EE continued working the utility position until being placed on a 5-pound restriction by an orthopedic specialist on 11/22/2023.
Employee tripped while walking along a supply train, slicing left hand on a piece of metal banding as EE attempted to break the fall.
Employee stepped off an approximately two foot ledge and twisted their right knee when they contacted the mine floor.
While handling steel set supplies, employee slipped and struck EE's tailbone against the heel of EE's boot. Injury was not the result of a training or compliance issue.
Employee had knelt down to check the temp on a bearing and felt a stabbing pain in EE's back. EE got self using guarding to climb onto a concrete curb. EE walked took about 3 steps, and when EE went to step off the curb, EE tripped and fell forward. EE latched onto the guarding to break EE's fall and when EE did, EE's back twisted, causing a tingling sensation in EE's buttocks.
Employee was trying to move a partially filled portable sand blasting unit that is on a dolly-type frame. When EE tipped the unit back on it's wheels, the weight of the unit knocked EE to the ground. The handle of the unit landed with force on EE's left thigh, causing injury.

Employee was coming down the steps from the school house elevator, when EE tripped on the bottom step. EE put hands out to catch self and pinched the right pinky finger between the lunch bucket handle and the bucket, resulting in a laceration to EE's right pinky finger that required stitches.
Employee was repairing the Plant Feed Head Chute while inside the head chute and slipped down. EE jammed EE's shoulder into EE's neck while trying to break EE's fall. Initial ER x-rays were negative for breaks. The employee seen by Orthopedics for a follow up and RTW on 4-25-2022. After a follow-up MRI revealed a fracture, the employee was written off work on Friday 5-6-2022.
EE was changing the cab filter on a 980M end loader. EE was positioned on the second step of the end loader ladder 34.5" off the ground. EE didn't maintain 3 points of contact.
Employee was standing on the track rail. Employee went to step up onto motor with right foot, when employee's left foot slipped and went under the step to the motor, causing an abrasion to left foot. The employee has a titanium plate, from a previous injury, in the spot of the abrasion. The abrasion became infected, causing the doctor to keep the employee off work.
Employee was loading straps onto the miner. Employee finished loading a set of straps and was returning to the scoop when EE tripped over a rock. As EE tried to catch self, EE felt a sharp pain from the right knee going towards the groin area.
Employee got off motor to throw switch and slipped in the mud, striking left shoulder on the ground. Employee worked until 8/31/2021.
The 8 Left belt went down with the head gate shields pulled in tight. Employee crossed stage loader to exit the face. As employee was coming down on the walkway side of stage loader, EE slipped and EE's leg got caught, twisting EE's left knee.
Employee was going to get back on EE's motor when EE tripped and fell forward, striking EE's left knee on the ledge, to the opening, of the operator compartment. Employee was not able to get an MRI until 5-9-2021 and results were read on 5-11-2021.
Employee stepped up into motor and felt a pop and stinging sensation in right knee.
Employee had finished changing miner sprays and was beginning to exit the top of the miner when EE slipped and put EE's hands out to catch self. EE's right hand caught a bundle of roof bolt plates, causing a laceration.
IE & co-worker were carrying a mounting plate for belt head. IE was walking backwards with the plate and tripped on a 2x2 piece of roof mesh causing IE to fall with plate landing on the right leg thigh area.
Employee was climbing down ladder of CAT 988 Endloader and felt a "pop" in left knee. The endloader was parked on uneven ground, making the last step longer than normal. Employee is fully trained and was wearing all required PPE.
Employee was on top of the mule "neatening up" the mule cable. When EE finished, EE jumped off the mule and landed awkwardly on the mine floor, injuring EE's right leg, just above the ankle.
Employee had just moved the loader cable when they stepped back a couple of steps into a hole, causing them to lose balance and fall. EE fell back on right hand, injuring right middle and right ring fingers.
A trained miner, with 11 years of total mining experience, was stepping backwards into a x-cut, to move away from loading machine, and stepped into a hole causing EE to twist and fall. Employee was wearing all PPE.
Employee was standing on the miner dust box ledge, cutting wire from around a bundle of bolts and lost EE's footing. As EE tried to catch self EE's right ring finger contacted a bundle of roof bolt plates causing injury.
Employee was operating a cement mixer at a crib plant. EE stepped off of the operator's platform and twisted right knee. Please note that the employee continued to work unrestricted until 11/2/2018. Officially off of work 11/5/2018.
Employee was shutting door on back of trailer, tripped over a crib block of wood and fell down.
Employee lost footing, tried to catch self, and wedding ring got caught on a bolt on the windshield of motor. This resulted in skin tearing. The finger was later amputated, at the hospital, because of a lack of circulation in the finger.
Employee was assembling the longwall stage loader on the 10 Right set up face. As employee was tightening a bolt employee's crescent wrench slipped and employee's body fell forward causing left forearm to contact a 1/4" staple.
While working as a pumper installing cans in the longwall entry, the injured employee turned to hand a pogo stick to someone and felt a pop in knee. EE was transported by ambulance to the hospital with a sprained knee and has not returned to work.
Employee was on overtime. EE stepped in a hole in the haul road injuring knee. EE did not seek treatment until 10/10. EE followed up with a Occupational Health Facility on 10/12 (next available appt.) & was granted a full release. EE worked the next two scheduled shifts. EE returned to the ER for re-evaluation on 10/14 & was place off work pending an MRI.
Employee was climbing into the walkway of a fletcher dual boom bolting machine. The machine was positioned on a downward grade making the walkway entrance higher than normal. EE fell while entering, striking EE's right knee on the walkway grating.
Employee fell & injured EE's shoulder while bolting on 3/19 but didn't report injury until 3/24. EE was seen by local ER dr. & continued working until follow-up appt. on 3/31. EE was placed on modified duty on 3/31 pending an MRI. EE was seen by an ortho dr. on 4/22 & it was determined EE would need surgery. This will be a charge back incident with first reported day missed 4/1.
Employee twisted knee exiting coal pile behind miner. EE was taken to hospital & issued full release. EE continued working EE's normal job, & is still working, but had fluid removed from the knee on 3/26/16. The removal of fluid constitutes more than normal first aid therefore making the incident medically reportable.
Employee exited mantrip to throw track switch. Employee slipped on the track switch cover and fell. Employee stuck employee's arm out to break employee's fall and dislocated employee's left thumb.
Employee slipped in the shower & sliced his right middle finger on a metal soap dish.
Employee was bolting off the company #3 walk through bolter. He began to fall. He reached out with his right hand and grabbed the machine to help break the fall. His hand became wedged and the force of the fall caused injury to his hand.
Employee was dragging a 1" rubber hose while injecting cement into a pumpable crib. Walking back to crosscut 89, employee tripped on a piece of banding material. Falling face first to the ground, the majority of the impact to the right side of the body.
Employee was throwing a piece of ventilation tubing over the miner head and slipped and fell, fracturing his left arm.
Employee got on bed of tractor trailer. Walked around the bed to hook up a load and slipped on icy surface and fell off of the truck. He has fracture elbows, fractured wrist, contusion of knee.
Loader operator was exiting the loading machine when he tripped over his loader cable and fell. When he fell, he caught himself with his right hand and strained his right shoulder.
Employee was dismounting the chain conveyor area of an Auxier Welding shuttle car. His right foot became entangled in a chain flight, twisting his ankle.
Employee was bolting off a Joy 14-09 mining machine. He stepped over some gob and hyper extended his left knee.
EMPLOYEE WAS REMOVING 6 FT. SECTIONS OF OLD TRACK RAIL FROM AN AREA OF THE HAULAGE THAT HAD BEEN REPAIRED. AS HE AND HIS BUDDY WERE WALKING FROM TRACK ENTRY INTO CROSSCUT, HE TRIPPED AND BEGAN TO FALL. HE STUCK HIS LEFT ARM OUT TO BREAK FALL AND HIS THUMB CONTACTED A METAL RAIL TIE CAUSING A LACERATION.
