Employee tripped on wash down hose while walking landing on left shoulder/arm resulting in a sprain. Cause of this incident was lack of awareness. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency was a factor.

Employee stepped in a hole resulting in EE spraining ankle. Cause of this incident was employee not paying attention to the surroundings. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency may have been a factor.

Employee was unloading material from scoop car onto supply cars, doing so EE tripped resulting in a sprained wrist while catching self. Cause of this incident was lack of EE concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.

Employee was stepping off stage loader onto the ground approx. 18" when EE rolled ankle. Cause of this incident was lack of concentration. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor, miner proficiency was a factor.

While helping another employee who had fallen, the injured employee fell and struck knee against a block, resulting in a laceration.

EE was carrying a belt roller cradle, while doing so EE tripped causing EE to drop cradle onto top of foot. Cause of this incident was lack of employee concentration. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.

Employee tripped when exiting the golf cart the employee was riding in. Employee landed face first on the ground. This causing a constant, sharp pain in lower back and bruising on lower lip.

Employee was hand spreading potassium permanganate onto pond for treatment when he slipped and fell on rocks getting some of this potassium permanganate into his eyes. EE seeked medical attention and received eye drops. PPE was not a factor, Compliance with rules/regulations was not a factor, Mining equipment/mining systems was not a factor, Miner proficiency was a factor.

While walking thru last open x-cut on at the longwall employee stepped in a hole twisting his right knee, employee began missing work on 12/17/15 as a result of this injury. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/mining systems was not a factor, miner proficiency was a factor.

Employee was climbing down a ladder to unhook ladder when he lost his footing causing a fracture to his lower leg. Cause of the injury was miner not paying attention to the task at hand. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency may have been a factor.

Employee was checking sensor at a pond site when he slipped and fell, attempting to catch himself he injured his shoulder. Employee began missing work on 6/29/15 as a result of his injury. Lack of awareness was the cause of the accident. PPE was not a factor, compliance rules/regs was not a factor, mining equipment was not a factor, miner proficiency was a factor.

Grating measuring 20"x42" had been removed from floor to perform scheduled maintenance,injured employee was walking back to area to perform work,he stepped into hole and struck his ribs against grating. Cause of injury was employee's lack of communication. PPE was not a factor,mining equipment/systems was not a factor,compliance rules/regs was a factor,miner profiency was a factor.

Injured employee stated he was moving out of the way of a scoop car when he tripped on some blocks causing him to fall and strain his groin.

Employee was getting out of a scoop on south A track. The employee slipped and fell catching himself with his left hand resulting in a fractured wrist. The cause of the accident was the employee's lack of awareness to his surroundings. PPE was not a factor, mining systems may have been a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

Employee was helping load supplies stepped off of supply car on uneven bottom and sprained his right ankle. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

The injured employee stated he was unloading roof bolts from a car when he slipped and fell off the car catching himself with his right arm causing pain to his right shoulder area.

Employee was walking thru x-cut when he tripped and fell landing on his left side, accident occurred on 5/19/14 but didn't become a RLTA till 5/24/14. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

Walking down overcast ramp and rolled ankle. PPE was not a factor,compliance rules/regs was not a factor,miner proficiency was a factor.

During a part 50 audit, it was determined this injury was reportable due to physical therapy. Employee stepped on a crib laying on the mine floor while carrying material. Cause of this injury was crib laying on floor and inattention. Compliance with rules/regs, PPE, mining equip/systems was not a factor, miner proficiency may be a factor.

Injured was helping to com-a-long a plastic waterline to move it and it broke causing him to step backwards in a hole twisting his left ankle. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner knowledge and proficiency was a factor.

During a part 50 audit, it was determined this injury was reportable due to a fractured toe. Employee went to move a spool of cable at a corner, overhanging brow fell striking his foot. Cause was not detecting loose top. Compliance with rules/regs, mining equip/systems, PPE not a factor. Miner proficiency may be a factor.

Injured was carrying a piece of belt structure off the rib, turned to take it to the belt stepped in a hole fell striking the back of his head to the ground causing an abrasion to his head. PPE was not a factor, mining equipment system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Injured was hooking up flinger duster to scoop to take off hydraulics. He had position himself between scoop and flinger duster. He got himself between the scoop and fell on belt structure laying on the mine floor injuring his left knee. PPE was not a factor, mining equipment/system was not a factor, compliance of rules/regs was a factor, miner proficiency was a factor.

Injured was walking by a parked scoop car tripped on a loose track tie fell and hit his front tooth on the top of the scoop car causing his front tooth to be knocked out. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency was a factor.

Injured was carrying a 8 inch water pipe with another person, he fell down water pipe hit on the right upper shoulder. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was loading 60" belt cradles into Ram Car. Employee stepped on a rock, fell down and twisted right ankle. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Employee was tied with a harness off of a monkey face welded to an I-Beam, the weld broke free allowing him to fall 8 feet to the ground. His head struck the ground cutting the back of his head. PPE was not a factor, compliance rules/regs was a factor, mining equipment/systems was not a factor, miner proficiency was a factor.

Injured was going through a mandoor that had a good bit of air, he slipped trying to go through mandoor, grabbed door frame and hit his thumb snapping it back. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Injured is now off work 8/16/2013. Scoop car was dragging plastic water line pipe, the pipe went sideways and hit right knee knocking him to the mine floor. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was bending a roof bolt his foot pivoted and slipped twisting right knee causing him to fall. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner knowledge was a factor.

Employee was removing a block wall. He pulled on door frame that was partially installed in the block wall and block started to fall. He was trying to run from the falling block and tripped backwards striking back of head and back. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Employee was walking along belt line to change belt rollers. While walking he slipped on uneven mine floor rolling his right ankle and fell to mine floor. PPE was not a factor, compliance rules/regs was not factor, mining equipment/system was not a factor, miner proficiency was a factor.

Employee was walking at the face area in mud that is ankle deep, got his right foot stuck in the mud causing him to fall. Causing a strain on his ankle. PPE was a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency was a factor.

Walking towards overcast ramp at belt head, tripped falling on right knee. Sprain knee. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, mining proficiency was a factor.

EE was walking towards ice, and fell on a patch of ice. His head made contact with the surface.

Employee clocked out after finishing his afternoon shift of work, was walking across parking to get in his car and slipped in the parking lot. He fell landing on his left ankle and ankle swelled up. PPE was not a factor, compliances rules/regs was not a factor, mining equipment/system was not a factor, miner knowledge and proficiency was a factor.

Injury occurred while contract labor was watching trailing cable on a shuttle car. The operator of the shuttle car reeled the cable and caught the leg of the injured took his feet our from under him he landed striking the back of head. PPE was not a factor, mining equip/system was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.

Employee tripped on a grade and fell on his knee. The results of the MRI along with a medical opinion dictated on 10/31/2012. The diagnosis was tearing of the meniscus and the employee is expecting to lose time.

EE was cleaning Prep Plant floors and steps, started down a set of steps while cleaning with water, the third step from the bottom had a bend in the step on one side. He slipped on it, fell on left side. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor. Became a lost time injury on 9/13 2012.

Employee was walking with cable during power move, Employee was going to put the end of the cable on a wooden skid. He slipped and fell over wooden skid fracturing his right leg. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was helping to build a block wall. The wall did not get wedged in as they were building the wall. The wall fell over as he stumbled backwards he tripped over the block that was used in building the wall, supposedly striking head on ground. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.

Employee was walking toward section power center slipped and fell on the ground. When he fell on the ground the employee landed on an old piece of a razor knife cutting his left hand. PPE was not a factor, rules/regs was not a factor, mining/systems was not a factor, miner proficiency was a factor.

Alleged incident was to have occurred while this contract labor was walking to the section from the supply car area and claims to have stepped in a tire rut, twisting his ankle. Cause of this injury was not paying attention to bottom conditions. Compliance with rules/regs, mining equip/systems, or PPE was a factor. Job skills and miner proficiency may be a factor.

Alleged injury was to have occurred while this contract labor was building a block wall while standing on a ladder, the wall started to fall over, knocking him off the ladder. Cause of injury was not securing wall with wedges as being built. Compliance with rules a factor/regs not a factor, mining equip/systems or PPE a factor and job skills and miner proficiency may be a factor.

Walking out of slope tunnel slipped in ditch area twisting knee. PPE was not a factor, compliance rules/regs not a factor, mining equipment/system was not a factor, miner concentration was a factor.

During a Part 50 audit, it was determined that this injury was reportable. Employee was pulling cable off a shuttle car and slipped, injuring his back. Cause was mud and uneven bottom with inattention to task. Compliance with rules/reg., PPE, mining equip./systems not a factor, miner proficiency may be a factor.

Walking down long wall face and lower back gave out. PPE was not a factor, Compliance of rules/regs not a factor, mining equipment/system not a factor, Cause is not being aware of mine floor conditions when walking on long wall face.

EE foot slipped on the rail as purat moved forward his foot got caught in between the ramp structure.

EE was assisting the Paurat as it was backing up the slope. Hoist rope got hung in the bridge conveyor when it broke free, hitting EE in the shin, flipped him up and onto the ground.

Walking toward elevator slipped on uneven mine floor falling and putting pressure on right ankle. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.
