Employee was fire bossing at 72 brk. 2 South seal pump when he tripped over a 6 inch discharge line and fell into a sand prop that was set around the seal wing wall injuring right knee.
Employee was walking toward face from the power center on the 6 West section. After the employee got past the power center about a break, he slipped on the wet roadway and twisted his lower back.
Employee was going to help to unload vulcanize equipment at 3 South Head. In the process of stepping out of the battery motor he stepped on a rock, twisting his left knee, and fell face first to the ground.

When the employee stepped in the insert he slipped on a 60" bottom roller causing him to fall, twist his right ankle, and to drop the 60" top roller on top of him.
Stepped on step of fuel tank on truck, falling resulting in broken leg.
Employee was walking around the power center on 19 HG Section. In the process of doing this, employees light cored got caught around the disconnect handle on the power center. This caused the employee to trip, striking his face on the power center. Employees tooth was knocked out.
Employee was dragging shuttle car cable by hand when he tripped over a piece of belt laying on the mine floor causing him to twist his left knee and ankle.
EE was hanging bolter cable across entry when he twisted his right knee. EE continued to work and was taken off work on 4/1/2015 to repair a torn ACL to his right knee.
Employee was moving miner cable out of the roadway. Employee picked up the cable and was walking backwards pulling the cable into a break through. The employee tripped and fell over a pressure pump striking the right side of his lower back on the corner of the pressure pump.
While surveying on the refuse embankment, victim twisted ankle and fell. Victim missed work following day to go to Health Care Clinic. X-ray indicated no breaks or cracks. Dr applied ace bandage and prescribed soaking in ice and ibuprofen specific follow-up visits and no restrictions.
Employee was carrying LW shear bits to the shear when he slipped on #5 LW shield relay bar, hurting his left ankle.

Employee was untangling an emulsion hose when he pulled on it and fell to the mine floor straining his back. *Incident became reportable on 5/23/13.
Employee and a fellow was cleaning a tailpiece with a water hose. EE jumped up on the tailpiece to keep from getting wet. When he jumped off the tailpiece he felt a pop in his ankle. He felt pain but continued to work and finished the shift.
Employee stood on a piece of 6" water line to hang a piece of chain on a belt hanger and slipped and fell spraining his right ankle.
While changing belt rollers EE was carrying a roller to change when he stepped on uneven ground curling both ankles and falling down.
EE was walking along the longwall face and stepped into a depression in the mine floor causing him to fall onto his left wrist. EE was not diagnosed with a sprained wrist until 5/31/12 at which time a cast was applied for additional support.
EE was walking up steps to guard house, tripped and fell, fell onto porch, hitting her right shoulder.

Walking out of security building when he stepped off last step onto ground - slipped on gravels and fell.
EE was pulling mine spray lineout of water hole. While pulling he stepped on wooded bridge by the hole. His foot slipped and he fell backwards. When he tried to catch himself his right hand struck something lacerating his middle and ring fingers. It could not be determined as to whether the bridge or the rock caused the laceration.
Employee was exiting a personnel carrier when the tip of his boot caught the edge of the personnel carrier causing him to fall. This incident did not become reportable until 11/29/10.
Employee allegedly fell on a flat car while unloading trash from it.
While trying to exit the elevator the employee fell onto the elevator floor causing him to fracture his left foot.
EE was walking around an end loader on the surface. He slipped on some ice and fell. This caused him to fracture his right foot.

Employee was retrieving splitsets that had been dropped into the sump area. When he was returning up the ladder to mine level, he untied and missed the hook off and fell back.

HE SLIPPED ON A COAL PILE, HIT HIS HEAD ON CRUSHER MOTOR.
Pulling belt and slipped and fell. When he fell, he landed on his hand. He continued to work until 02/23/04.
