Showing all 5On 08/20/21 the employee working on grating knelt to perform assigned task. When repositioning body they twisted their knee in the grating rather than standing, turning and then kneeling a second time. On 08/23/21 the employee requested medical evaluation and was issued a work restriction.
The employee was in the quarry electrical room (ER-16) to de-energize and lockout a piece of equipment. While walking to the cart to leave the quarry, the employee tripped on a rock and twisted ankle.
On November 29, 2019 the employee was kneeling to replace a seal on the 732PP01 pump on the solvent pad. When the employee stood up, EE felt EE's knee give out. On December 3, 2019 EE reported that the injury had gotten worse. EE was placed on restricted duty on December 4, 2019.
On 11/14/18 while exiting the dozer the employee stepped on a lump of marl and twisted an ankle. On 02/25/2020 the employee had surgery on the ankle from injury that occurred on 11/14/18.
