While cleaning in the 010 Conveyor area, employees noticed unusual material on the floor, but did not think it was concerning, assuming it was safe since spills or hot materials are normally barricaded. As one employee stepped to retrieve a shovel, the material gave way, and hot material entered their boots, causing a second-degree burn.

Showing all 12While walking down a grade beside a conveyor EE slipped on a mill ball. EE lost balance and caught self with the same leg twisting R knee. EE felt knee pop. EE immediately reported this to supervisor. EE did not see a doctor until May 11 when knee pain was still present. On November 5 MRI revealed cartilage tear and had surgery on December 8.
Employee had been working for a couple of days on a dozer, kneeling and crawling on the metal tracks of the dozer. He developed a sore spot or rubbed the skin from his left shin causing infection.
While the worker descended a ladder from silo #7, he stepped on a rock that was on the platform below rolling his right foot. Shift started 10 p.m. July 8, but accident did not occur until 12:30 a.m. July 9.
While walking past a bandsaw in the Maintenance Shop, worker stepped on a 12" piece of threadstock in the aisle causing him to turn his right ankle and sprain it.

EE WAS INSPECTING EQUIPMENT ON GROUND FLOOR OF THE MILL BUILDING WHEN HE STEPPED ON A SMALL STEEL GRINDING BALL THAT WAS COVERED BY MATERIAL AND NOT VISIBLE, CAUSING HIM TO TWIST AND STRAIN HI S BACK. DID NOT REQUEST TO SEE DOCTOR UNTIL 3-20 WAS RETURNED TO FULL DUTY AT THAT TIME, HOWEVER ON VISIT OF 3-27 DOCTOR PRESCRIBED PHYSICAL THERAPY TO COMMENCE WEEK OF 3-31-03.
EMPLOYEE WAS USING A FORKLIFT TO MOVE BRICK PALLETS. WHEN HE GOT OFF OF THE FORKLIFT HE STEPPED ON A ROCK TURNING HIS ANKLE. THIS WAS EMPLOYEE'SFIRST SHIFT RETURNING TO WORK FROM THE PREVIOUS LOST TIME INJURY. THIS INCIDENT WAS A RE-INJURY RELATED TO PREVIOUS LOST TIME INCIDENT.
EE WAS COMING DOWN STEPS FROM C15 AND STEPPED ON A ROCK AT THE BOTTOM OF THE STEPS CAUSING HIM TO TWIST HIS ANKLE. ANKLE GOT PROGRESSIVELY WORSE AND EE ASKED TO SEE CO DR ON 1-25-01. CONTINUED TO HAVE PROBLEMS AND STARTED LOSING TIME ON 1-29-01.
WHILE WALKING ACROSS GROUND FLOOR OF MILL BUILDING EE STEPPED ON A MILL BALL THAT WAS UNDER APPROX. 3" OF DUST ON THE FLOOR. EE DID NOT REQUIEST TO SEE A DR. UNTIL 12-1-00. SUBSEQUENT TESTS RE VEALED NOTHING, HOWEVER HE CONTINUED TO HAVE PAIN, SO A DIAGNOSTIC ARTHROSCOPY WAS SCHEDULED AND PERFORMED ON 2-28-01, WHICH WILL RESULT IN LOST TIME.
EE WAS WALKING IN CLINKER TUNNEL WHEN HE STEPPED ON SOME LARGE CHUNKS OF CLINKER CAUSING HIM TO TWIST HIS KNEE. DID NOT REQUEST TO SEE COMPANY DOCTOR UNTIL 4/12/00. STARTED LOSING TIME ON 5/3/ 00 AND WILL ULTIMATELY HAVE ARTHROSCOPIC SURGERY.
