EMPLOYEE CUTTING NIPPLES OUT OF 2" PVC AND PLACING THEM BESIDE PICKUP WHERE CUTTING OPERATION WAS TAKING PLACE. CREW WAS STARTING TO PICK TOOLS UP BEFORE QUITTING TIME, APPROXIMATELY 3:30 P.M. WHEN EMPLOYEE STEPPED ON ONE OF THESE 2" PIECES OF PVC CAUSING EMPLOYEE TO LOOSE HIS BALANCE. WHEN EMPLOYEE TRIED TO CORRECT HIS BALANCE HE TURNED HIS BODY IN OPPOSITE DIRECTION OF FALL. THIS
