EE knocked a drain valve off piping at 2 SOF pump, liquor contacted lower leg area.
EE was cleaning the top of #5 Washer when he bumped his head on a metal beam. He was going under the beam and raised his head before he cleared the beam. Neck Strain
Employee turn to move to another spot and hit his left knee on a value. Employee did not become reportable until 12/1/14 when he went to the doctor and received restriction. Left knee contusion.
EE had just completed tearing down 3-17 heater. As he was moving his equipment out from under the heater, his glasses fogged up. EE was walking, looking down, and hit his head on the blow down header piping. Cervical Strain
Employee trip over pipe and scrapped leg. Contusion to right lower leg.
Working at 1-2 Desilicator and was dragging a hose and saw a pipe above and ducked to go under the pipe. EE did not see a second pipe and hit his hard hat on it and then fell to the floor. neck strain
EE was replacing the valve handle on feed pump. As EE was walking away from the pump he struck his head on an overhead support beam causing him to jam his neck. On 01/22/13, EE was prescribed a muscle relaxant and 2 weeks of physical therapy, making this a reportable injury. No lost work days or restricted duty.
EE was working under a piping flange and as he stood up he struck his hardhat on the piping flange jamming his neck. There was a mound of scale build up creating an elevation change. The EE claims that the scale buildup is what caused him to strike his head on the piping flange. EE on restricted duty from 12/22/2011 to 01/03/2012. EE RTW full duty on 01/02/2012.
On 11/17, the EE struck his head on an overhead structure. EE reported the incident to mgmt on 11/18. EE was seen by the medical staff & RTW w/o restrictions. On 11/25, EE reported to the medical facility complaining of dizziness, pain & tingling in his left arm. EE was sent home. EE did not report to work on 11/26 & 11/27. On 11/28/, EE placed on restricted duty by his Dr.
EE stepped off a concrete pad and hit his heel on the lower pad. EE stated that he didn't realize that it was that much of a grade when he stepped off hitting on the heel of his left foot. Initially diagnosed as a contusion, on 6/16/11, MRI revealed a hairline fracture on his left heel. On 6/17/2011, he was placed on restricted duty.
