Putting cutting edges on d8r. using a putty knife scrapping off mole board. A spur on the edge cut the left index finger on the side. Having to put 5 stitches on the side.

A roof fall was found on the 11-4-05 in the #6 entry the southeast mains at spad #26 & 35. Fall was 4' high, 18' wide, 40' long. 48" fully grouted roof bolts were used in this area.

Fall, took out a section of grating to access shoot that was cloged. EE forgot about it and stepped off into the hole. He caught himself with his arm and cut it on a piece of grating.

A ROOF FALL WAS FOUND AROUND 6:05AM THE 11TH IN THE #8 ENTRY OF THE MAIN SOUTHEAST ENTRIES AT SPAD #0190 BY THE PRESHIFTER. THE FALL WAS 20' WIDE, 60' LONG AND 48" HIGH. DOUBLE LOCK 48" BOLTS WERE USED IN THIS AREA. THIS WILL NOT BE CLEANED UP

A roof fall was found in the main south supply road #5 entry between spad #31 + 50 & spad #31 + 95. The fall was 4' or 5' high, 18' wide & 40' long. 4' double lock roof bolts were used in this area.

EE WAS ROOF BOLTING. WHEN HE SET HIS BOLTER UP TO PUT UP A ROOF BOLT AND AS HE PUSHED THE ROOF BOLT UP AGAINST THE ROOF, A PIECE OF ROCK BUSTED UNDERNEATH THE BOLTER. THIS CAUSED THE BOLTER TO DROP DOWN 4-5", STRIKING THE EE ON THE HEAD WITH THE CANOPY.

Injured party was beside the scoop where he had been loading blocks into the scoop bucket. He had his right hand lying up on the scoop bucket behind the stinger when the operator shut scoop off & accidently hit the lever with his knee getting out of scoop. The scoop bucket came back & caught the ring finger on the right hand at the end of the finger.

EMPLOYEE & CO-WORKER WERE CHANGING A TRACK ON 151-D-11. THEY HAD 3 BOLTS LOOSE & THE 4TH BOLT WOULD NOT BREAK LOOSE. EE WAS USING A TORCH TO HEAT THE TRACK LINK & A TRACK PIN FILLER PLUG BLEW OUT OF THE PIN, BLOWING HOT OIL ON THE RIGHT SIDE OF HIS FACE.

EE OPERATING A MUD CAT DREDGE. CLEANING OUT SILT BASIN BELOW THE COAL YARD. EE THOUGHT THE HEAD WAS TRYING TO STOP UP, GOT OUT OF CAB TO TAKE A LOOK. PUT THE AUGER IN NEUTRAL AND LEFT THE ENGINE IDLEING. WALKED TOWARDS THE HEAD TO GET A CLOSER VIEW, AND CLIMBED OVER OIL THROUGH HANDRAILS, SLIPPED AND WENT INTO AUGER. LEG HAD TO BE REMOVED BELOW KNEE.

EE WAS HELPING FROM THE BANK TO GET SUPPLIES TO MEDICAL PERSONNEL. GROUND CONDIDITONS WERE MUDDY AND SLICK. EE FEET SLIPPED OUT FROM UNDER HIM, CAUSING HIM TO FALL AND LAND ON HIS RIBS. EE WAS COMPLAINING OF POSSIBLE RIB FRACTURE.

LOADER OPERATOR NEVER SAW EE & KEPT GOING, EE ROLLED BETWEEN FRONT LOADER TIRES, FRONT LINE EDGE RAN OVER EE'S LT. ARM FRACTURING BONE.

As ee attempted to install a cable trip system to a breaker that was energized on the top side, he inadvertantly stuck part of the bracket into the energized wire causing flash burns to his hands and neck.
