Showing all 5Tripped and fell on upper left arm. Employee DX was humeral fracture. Returned to work on 2/15/13. Crockett County Mining paid employee regular wages until returned to work as well as all medical bills. Incident was reported to Texas Mutual as report only. No comp claims filed.
Employee was working on a repair on the plant when the return roller slipped and mashed left middle finger. He was taken to the ER. Finger was broken but returned to full duty on 6/11/12.
Operator slipped on top of excavator while refueling and injured shoulder. Operator worked next few days then went for an appointment and local provider. Local provider placed employee on rehab. Surgery was required 2 months later.
Pit truck was dumping feed material into receiving hopper. Pin on bottom of lift cylinder broke causing dump bed to shift to one side and turning truck over. Operator was not wearing seat belt. Truck was totaled & operator had a broken arm. Marijuana was found on operator at the hospital and may have been a contributing factor.
