EE was screening coal and a rock jammed in the feed belt. EE stopped the screener. He then climbed onto feeder. He rolled the jammed rock, Another rock rolled and hit his left ankle. EE was wearing hard hat, safety glasses, steel tip boots and gloves.
EE was screening coal and a large rock jammed at head wheel. Rock was released from head wheel and lodged at chute at end of vibrator. EE then SHUT THE MACHINE DOWN. EE tried to roll the rock loose and the rock pinched his finger, breaking 2 fingers. It broke the index and middle fingers.
In entrance #3 (beltline) the conditions were wet and sloppy, no bottom had been cut and the floor was slick. EE slipped and twisted right knee resulting in torn mcl and miner cartilage damage.
Area was about 38' high causing use of different drill steels.
EE WAS SHOVELING COAL FOR FURANCE AND LID OF BILE WOOD FELL ONTO HIS BACK. EE SUFFERED BRUISED BACK SENT TO MEDICAL CENTER FOR EVALUATION.
LIFTING ENGINE OUT OF D-9 BULLDOZER, CHAIN WAS TWISTED DID NOT SEE IT. ENGINE SWUNG OUT OF MOUNTS KNUCKED ME OFF OUT NO DANGER OR IMPAIRMENT-BRUISED LEFT ARM.
