Independence Excavating · Struck by falling object

The crew was working on pulling power cables getting the plant set up. When two employees were pulling cable through a hopper extension. One employee was pulling cable with their hand resting on the back of the conveyor. The hopper extension slipped out of workers causing it to fall onto the resting employees fingers lacerating their middle, ring, and pinky finger.

At 3:30 am a crushing operator came in contact with the crusher housing causing a laceration to EE's right finger.

While attempting to change out out a tire on a skid loader. the operator got left finger in between the rim of the tire and hub of the machine while mounting the tire. The operator immediately felt pain in the left finger when EE removed the glove and saw a laceration on left finger.

Loader operator was pulling clinker material and the right front tire of the John Deere Loader caught on fire. The employee used a Fire Extinguisher - The local Fire Dept was called to put out the Fire - There were no injuries due to the Fire

Maintenance employee was replacing a skirt rubber on a chute above the main feed belt. The employee did not Lock Out the feed belt and while working on the chute a piece of metal fell onto the moving belt, the employee tried to grab the metal and caught his right arm between the belt and belt cover. The employee sustained a fracture to his right wrist and forearm.

A maintenance employee was reworking rubber belting in the finish screen tower when a spark caused the belting to catch fire - The employee utilized a Fire Extinguisher and was unable to put out the Fire - The local Fire Department was called and put out the fire. There were no injuries due to the Fire.
