Slippery surface due to snowy conditions. Employee was stepping down to another level of the catwalk and slip and fall on to the catwalk
Cleaning out Gabion Grizzly, Rocks get Jammed and emplyee has to either pry or push the rocks out of the grizzly. Employee was using a sledge hammer to get the rocks lose and pulled something in his right shoulder.
Climbing down a ladder, stopped to re-hook safety chains. Stepped on power cables hanging against the ladder. Fell appox. 4 ft to 5 ft injuring right shoulder.
2 EE's were changing out a broken drag cable on the 4600 manitowic. EE was installing the new drag cable into the crow foot assembly. On the underneath side of the plastic lined cable were two pieces of cable out of lining, which the EE did not see. Upon pulling on cable, the piece cut his left index finger. His hand slipped off the cable, cutting his L index finger.
The employee was shoveling dirt when he felt a pop and burning feeling. It resulted in a rotator cuff sprain.
The terrain was rough with rocks strewn about the site. The employee was manually pulling slack from conveyor belting when his knee slipped sideways and something popped inside his knee.
Employee was exiting a track excavator and stepped on a rock and his right knee gave way.
Slipped on ice carrying heavy object. Received 5 stitches below left knee, contusion, and strain.
While employee was trying to start the primary feeder, the bar he was using slipped, he lost his balance & fell off the frame.
Left 2nd middle finger required sutures.
Foreign matter in Right Eye, Doctor had to remove.
EE STEPPED OUTSIDE THE HANDRAIL TO CLEAN HIS WINDSHIELD AND HIS FOOT SLIPPED CAUSING HIM TO FALL INTO THE TURN SIGNAL.
WHILE CHANGING SCREENS ON THE WASH PLANT, A PLASTIC PIN BROKE AND A FRAGMENT BROKE HIT EE IN THE EYE. EE WAS NOT WEARING EYE PROTECTION AS REQUIRED BY OUR SAFETY PROGRAM. CONJUNCTIVAL CONTUSIO N TO THE LEFT EYE.
EMPLOYEE ALLEGES THAT HE WAS SHOVELING AROUND THE PLANT WHEN HE SPRAINED HIS RIGHT SHOULDER.
THE EE ALLEGES THAT HE WAS LIFTING A LARGE ROCK THAT WAS LODGED IN THE STEEL BARS OF A JRIZZLEY WHEN HE STRAINED HIMSELF.
