EXAMINER DISCOVERED AND UNPLANNED ROOF FALL IN THE #3 BELT ENTRY BETWEEN X-CUT 8 & 9. THE FALL WAS NOT IN AN ESCAPEWAY OR IMPEDE VENTILATION. THE FALL WAS APPROXIMATELY 50'-70' LONG, 20' WIDE & 7' HIGH. THE ACCESS TO THE AREA WAS DANGERED OFF AND 2 ROWS OF BREAKER POSTS WERE SET.

We had an unintentional roof fall in 3rd West. It was in #4 entry and inby XC 24. The area is not a working section. It did not affect ventilation or involve an escapeway.

The weekly examiner discovered a roof fall located between XC 24 and 25 in the return entry of A-West. The area had supplemental roof support installed due to the faulted area. There was no weight on cribs, timbers or bolts. The entry had been traveled within the last week for EO1 inspection. The roof fall closed off the escapeway that would be used by the Main West section.

EE was working on conveyor chain on miner. He was swinging a 4 lb hammer and slipped. When he fell he landed on his left side. He complained of hip pain.

The subject was bolting. He was trying to get a drill steel, that had become stuck, out of the roof. The steel came out and struck the subject on the left pinky finger. He had to get 9 stitches.

THIS WAS IN AN AREA THAT WAS BOLTED WITH 5' BOLTS PRIOR TO THE OPERATOR VOLUNTARILY CHANGING TO 6' BOLTS. THE AREA WAS DANGERED OFF TO PREVENT UNAUTHORIZED ENTRY.

THE FALL WAS LOCATED IN AN AREA WHERE 5' BOLTS HAD BEEN INSTALLED, PRIOR TO THE OPERATOR VOLUNTARILY USING 6' BOLTS.

IT SHOULD BE NOTED THAT A 6.3 EARTHQUAKE OCCURRED ON THE UTAH-NEVADA BORDER AT THE SAME TIME WHICH WAS FELT THROUGHOUT MOST OF THE WESTERN STATES. THE HORIZON MINE BELIVES THIS FALL WAS A DIRECT RESULT OF THE EARTHQUAKE.
