EXAMINER DISCOVERED AND UNPLANNED ROOF FALL IN THE #3 BELT ENTRY BETWEEN X-CUT 8 & 9. THE FALL WAS NOT IN AN ESCAPEWAY OR IMPEDE VENTILATION. THE FALL WAS APPROXIMATELY 50'-70' LONG, 20' WIDE & 7' HIGH. THE ACCESS TO THE AREA WAS DANGERED OFF AND 2 ROWS OF BREAKER POSTS WERE SET.
Subject slipped on hill side. Pulled muscles in left lower leg.
EE WAS THROWING BELT STRUCTURE ON A-WEST #3 WHEN FELT SOMETHING PULL IN LOWER STOMACH & A LITTLE LOWER. IT BURNED FOR A LITTLE WHILE & PAIN COMES & GOES.
Subject was holding a drill steel for another person. The other person swung a hammer and missed the steel and hit the subjects left hand.
Subject was cutting open calcium chloride bags and cut his leg.
Subject was cleaning out scrubber tank on LST - 5 scoop. The water hose slipped out of his hand and high pressure water hit his eye.
Pulling rock out of feeder. Subject felt something hit the back of his right leg.
EE WAS HELPING ROLL BELT, BENT OVER & PICKED UP SOME BELT CORES AND HIS BACK STARTING HURTING HIM.
Subject was walking down hill behind sub station and slipped on the ice and fell.
Subject was operating a IT loader and door slammed shut on his left hand. There was tissue damage.
Subject was running throttle of LST 5 scoop manually. Subject became wedged between scoop and roof. The subject developed a hernia and needed further care. Subject was operated on 12/13/2011.
The subject was picking up a rock in the roadway. The rock slipped and fell and hit him on the left foot.
Subject was making a splice on a shuttle car cable. While cutting the cable his knife slipped and cut his right leg approximately 1 inch long.
We had a roof fall in #6 entry between XC 10 and XC 11. This area had been showing signs on the 17th of July and the area was immediately cribbed and dangered off to keep people out of the area.
Subject was repairing a splice on a shuttle car cable. While cutting the cable with a razor knife, the knife slipped and cut his right leg.
Subject was dragging mesh to the bolter. He twisted his ankle when stepping on a piece of coal.
We had a roof fall. It was located at XC 11 entry 4 in 1st right. The area had been dangered off and cribs had been set on all 4 sides of the intersection before the roof fall occurred.
EE WAS ASSEMBLING RADIATOR, TURNED TO WALK AWAY, RADIATOR ASSEMBLY FELL, STRIKING EE ON RIGHT LOWER SHIN ABOUT 4" ABOVE ANKLE.
Subject was roof bolting in the face. The drill steel fell out of the hole in the roof. It hit his finger and caused a small laceration.
Subject was driving a shuttle car. A piece of coal fell and smashed his finger. He received 3 stitches.
Electrician was taking lead from a de - energized circuit breaker. The circuit breaker flashed and burned the electricians hands.
We had an unintentional roof fall in 3rd West. It was in #4 entry and inby XC 24. The area is not a working section. It did not affect ventilation or involve an escapeway.
We had an unintentional roof fall in #3 entry XC 23 in 3rd West.
Subject stepped out of the mantrip and rolled his right ankle.
The subject was walking down the bath house steps. The second step broke and the subject twisted his foot.
The weekly examiner discovered a roof fall located between XC 24 and 25 in the return entry of A-West. The area had supplemental roof support installed due to the faulted area. There was no weight on cribs, timbers or bolts. The entry had been traveled within the last week for EO1 inspection. The roof fall closed off the escapeway that would be used by the Main West section.
EE was working on conveyor chain on miner. He was swinging a 4 lb hammer and slipped. When he fell he landed on his left side. He complained of hip pain.
Subject changing a belt roller. He stepped back and rolled his ankle.
The subject was bolting. He was trying to get a drill steel, that had become stuck, out of the roof. The steel came out and struck the subject on the left pinky finger. He had to get 9 stitches.
Had an unintentional roof fall in Main West return at XC 4 . Did not impede travel or affect ventilation. Was reported to MSHA at 9:00PM
EE WAS LOADING FIRE SUPPRESSION BOTTLE IN MANTRIP, SMASHED FINGER REQUIRING SEVERAL STITCHES ON RIGHT FINGER.
EE WAS WALKING IN ENTRY, STEPPED IN MUD HOLE AND TWISTED LEFT KNEE.
EE HAD RIGHT HAND ON MESH HOLDING A ROOF BOLT IN LEFT HAND. OTHER OPERATOR RAISED BOLT AND PLATE SMASHED EE'S RIGHT MIDDLE FINGER REQUIRING 4 STITCHES.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE WEEKLY EXAMINER IN THE NORTH-SUB MAINS, #2 ENTRY, X-CUT 13. THE FALL MEASURED APPROX. 5.5'Hx20'Lx16'W. THIS WAS IN AN AREA THAT WAS BOLTED WITH 5' BOLTS PRIOR TO THE OPERATOR VOLUNTARILY CHANGING TO 6' BOLTS. THE AREA WAS DANGERED OFF TO PREVENT UNAUTHORIZED ENTRY.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #2 X-CUT, BETWEEN THE #1 & #2 ENTRIES OF 2ND NORTH (NORTH-SUB). THE FALL WAS APPROX. 40'L x 18'W x 5.5'H. THE FALL OCCURRED IN AN AREA THAT HAD BEEN BOLTED WITH 5' ROOF BOLTS PRIOR TO THE OPERATOR VOLUNTARILY INSTALLING LONGER ROOF BOLTS.
AT 11:30 AM, AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE BELT EXAMINER IN THE NORTH SUB-MAINS AT X-CUT #14. THE FALL WAS APPROX. 40'L x 16'W x 6'H. THE MSHA HOTLINE WAS CALLED AT 11:44 AM. THE FALL WAS LOCATED IN AN AREA WHERE 5' BOLTS HAD BEEN INSTALLED, PRIOR TO THE OPERATOR VOLUNTARILY USING 6' BOLTS.
EE WAS PUSHING DRILL STEEL UP IN HOLE, OTHER STEEL WAS IN CHUCK, WAS GETTING READY TO COMBINE STEELS, WHEN UPPER STEEL CAME DOWN AND CUT EE'S LEFT MIDDLE FINGER, REQUIRING 5 STITCHES.
EE WAS PULLING ROOF BOLTER CABLE, STEPPED ON A LUMP OF COAL, AND TWISTED HIS RIGHT ANKLE.
EE WAS CUTTING BRATTICE CLOTH WHEN KNIFE SLIPPED LACERATING EMPLOYEE'S RIGHT THIGH.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY MSHA INSPECTOR AND SAFETY DIRECTOR IN THE 2ND WEST MAINS, #6 ENTRY, BETWEEN X-CUTS 46 & 47. THE FALL MEASURES APPROX. 25'Lx18'Wx5'-6'H. MSHA WAS NOTIFIED AT 1:20 PM. THE AREA WAS DANGERED OFF.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, #5 ENTRY, BETWEEN X-CUT #14 AND X-CUT #15. THE FALL MEASURED APPROXIMATELY 22'L x 12'W x 6' TO 6.5'H.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, #5 ENTRY, BETWEEN X-CUTS #6 & #7. THE FALL MEASURED APPROXIMATELY 25'Lx18'Wx6'H. IT SHOULD BE NOTED THAT A 6.3 EARTHQUAKE OCCURRED ON THE UTAH-NEVADA BORDER AT THE SAME TIME WHICH WAS FELT THROUGHOUT MOST OF THE WESTERN STATES. THE HORIZON MINE BELIVES THIS FALL WAS A DIRECT RESULT OF THE EARTHQUAKE.
EE WAS HAMMERING ON A PIECE OF STEEL CABLE TO CUT IT AND MAKE A MOLLEY. A SMALL PIECE OF WIRE BROKE LOOSE AND PENETRATED THE OUTER SIDE OF EE'S LEFT HAND.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED AT 12:45 PM. MSHA NOTIFIED BY PHONE AT 12:57 PM. FALL WAS IN 2ND WEST MAINS, ENTRY #5, X-CUT #36. FALL MEASURED APPROX, 18'W x 20'L x 5' TO 6'H. AREA WAS DANGERED OFF WITH BREAKER ROWS, AS NO TRAVEL IS REQUIRED IN THIS AREA.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, #5 SECONDARY ESCAPEWAY ENTRY, BETWEEN X-CUTS 31 & 32. THE FALL MEASURED APPROX. 18'L x 16'W x 18"H. MSHA WAS NOTIFIED BY PHONE AT 9:42 AM.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, X-CUT #19 BETWEEN THE #4 AND #5 ENTRIES. THE FALL IS APPROXIMATELY 30'L x 18'W x 6'H.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS, X-CUT #19, BETWEEN THE #5 & #6 ENTRIES. THE FALL IS APPROXIMATELY 20'L X 12'W X 6'H.
EMPLOYEE WAS CROSSING OVER CONVEYOR BELT WHEN BELT STARTED. TRAVELED APPROX. 25' BEFORE BELT WAS STOPPED. SOFT TISSUE INJURIES TO BACK AND RIGHT HIP.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE 2ND WEST MAINS SECTION, ENTRY #5, X-CUT #37. THE FALL MEASURED APPROXIMATELY 20'Lx20'Wx5.5'H.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE MINE EXAMINER IN THE 2ND WEST MAINS, #6 ENTRY, BETWEEN X-C #9 AND XC# 10. THE FALL MEASURED APPROXIMATELY 60' L x 18' W x 4' TO 5' H. THE AREA WAS DANGERED OFF AND ADDITIONAL FLOOR TO ROOF SUPPORT WAS INSTALLED.
