Showing all 6The employee was looking at a welder and had propped the engine cover open. While looking at the engine the cover fell and fractured the employee's right wrist.
Employee was cutting on a boom on a Cat 749 Excavator with a torch. The torch hose blew a hole in the line, caught fire and burned the employee on EE's left forearm.
The employee was walking near the shop building and stepped on a loose nail on the ground. They were taken to an emergency clinic where the wound was cleaned, a tetanus shot was administered and given oral antibiotics. They were then released back to full duty on the same day.
Note: Form 7000-1 is being filed only to record the incident on our property (6/11/12). The dump truck driver (customer) was on our property to obtain a load of aggregate. He is neither an employee or independent contractor. While on our property in his dump truck, the individual apparently had a heart attack which resulted in his death.
EMPLOYEE WAS OPERATING A CRANE TO MOVE A GENERATOR THAT WAS POSITIONED ON THE BANK OF A SETTLING POND. WHEN THE EMPLOYEE MOVED THE GENERATOR WITH THE CRANE, THE CRANE TIPPED OVER. THE CAB WAS CRUSHED ON IMPACT WITH THE GROUND FATALLY INJURING THE EMPLOYEE.
