While working on North wall mitigation, dozer ( Cat D10T) slid down the dig face and rolled over landing back on it tracks. Employee was seat belted in and received small laceration to forehead and minor bruising to the shoulder.
Employee was pulling and straightening a large set of tire chains that were laying on the floor. A large hook bar was used to help with this. The hook bar slipped off the chain link which resulted in the employee falling backwards and spinning body around and landing on right knee cap on cement floor.
Exposure to live/energized circuit parts (line side of electrical breaker)
A Sample beaker of approx. 20 Ml of 70% nitric acid slipped out of her hands and struck the counter top, broke and splashed liquid on her arms and small amount on face.
Employee went over to the north side of the TM to remove a blank that had fallen out of the guides. While employee was pulling up on the blank he suddenly felt pain in his lower back.

Operator was dismounting fuel truck, facing away from the apparatus, yet maintained 3 points of contact. The operator's feet slipped off of the bottom rung causing his left back side to make contact with the hose reel. It was raining at the time of the incident.
Employee picked up core box full of core sample (25-30 lbs.) and sprained lower back
Employee was operating a Cat 785 haul truck with the door open and the door shut on his left thumb.
Employee was cutting hole in top of thread grease bucket when knife slipped cutting his left hand.
Employee was working on a Cat jack stand that was stuck in lower section of the stand. The injured employee was supporting the stand while another employee was using a pipe as a slide. The injured employee's finger accidentally went into a hole of the stand as the pipe slid into the stand striking the rt 2nd digit. The impact caused a partial amputation.
Employee was installing a 42" culvert pipe and received a laceration to his right thumb while he was positioning the pipe.
Employee was driving down haul road when his truck started gaining speed and the employee was unable to gain control of the truck causing him to hit the berm and became airborne - the truck impacted the bench below causing the truck to bounce and spin to the right resting on its tires.
As the driver was traveling out of the pit and onto the south haul road, the driver did not see a pot hole and drove his truck through the hole injuring his left neck and shoulder.
Employee was operating a Cat 793 haul truck and felt discomfort in his eye. Employee cleaned his contacts and finished his shift. The following day his eye was swollen and was taken for medical evaluation where he was diagnosed with a cornea abrasion.
The truck driver said a load of material was dropped into the bed of his truck too hard by the loader operator and it jarred his neck.
The haul truck driver was pulling into the pit to be loaded and hit a pot hole. This caused the truck to rock back and forth; the rocking of the truck strained his neck.
While placing 4" HDPE pipe into the fusing machine to be welded, the jaw that holds the pipe into place fell. It struck the middle knuckle of the miner's left middle finger causing a laceration.
Employee was dropping a cable tree and the locking rope pulled his hand into the pulley causing an avulsion to the left 5th finger.
Employee was being trained to operate a haul truck. The vent fan was turned on and dust was blown into employee's eyes which caused a cornea abrassion to right eye.

An accident occurred involving a 30 ton Artic truck #305. Icy road conditions caused the truck bed to slide accross the center berm of the road. The rear wheels of the truck bed caught on the berm and flipped the box of the truck over.
Employee bent over to clean level sensor, leaned on grating injuring a rib.
Employee was using a hammer/hatchet to remove nodule from a cathode, when the hammer kicked out, causing pain to the employees wrist. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 11-22-2010.

Employee was changing drill bits when his foot got caught on drill deck. Employee fell into handrail fracturing two ribs.
Employee was moving a 55 gallon drum with a pry bar. The pry bar slipped causing left 5th finger to get caught in a pinch point. The injury resulted in a fracture of the 5th finger.
Employee was changing bucket teeth on a track hoe. The employee hit a pin punch with a hammer and the hammer head chipped. The chipped piece struck employee in the right cheek.
Employee was being loaded with material when a boulder hit the windshield jarring the employee.
While unhooking a braided wire tow cable, pin slipped causing a loose wire to cut left index finger, cable was dropped. Lacerated finger required 14 sutures.
Employee was kneeling/crawling under a dozer to make repairs. He began to experience soreness after completing the work and gettin out from under the dozer. His knee began to swell. He was treated at a local clinic and released back to work the same day.
Employee was cutting expanded metal and saw kicked back coming down on employee's left thumb cutting thru his glove and into his thumb. Employee received 3 stitches.
EMPLOYEE WAS ATTEMPTING TO REMOVE A LIP PROTECTOR FROM LOADER BUCKET - HE WAS WORKING IT OFF THE LIP WITH A HAMMER AND A PIECE OF METAL CAME OFF STRIKING EMPLOYEE IN THE INNER THIGH.
Employee was loading plastic float shell into back of pickup when he felt a pop in his right elbow causing pain.
HUMAN FACTOR POSSIBLE MEDICAL EPISODE
Employee was walking across grind deck and tripped on a piece of grading. Employee fell on his knee cutting Left knee.
Employee was changing the tire on a skid-steer loader. As he was attempting to slide the tire off the lug bolts, he did not know that the tires where foam filled and the weight of the tires. When the rim slid off the lug bolts, the employees right middle finger got pinched between the rim and hub. Employee cut his middle finger, receiving six stitches.
Employee put pipe wrench on pipe, the driller rotated the pipe but wrench did not grip, so employee put right hand on pipe wrench jaw to force it to grip the pipe. Pipe gripped for a second then slipped causing the jaw to slam back into employee's right hand and wrist.
Employee was cleaning up 3" ball mill balls for the floor. He put a ball into the bin and as he was reaching down to pick up another ball, the first one fell out and hit his left ring finger pinching it between the two balls fracturing his left ring finger.
Worker had finished fueling a Cat 14h motor grader, while dismounting the machine using the steps, his right foot slip off the last step resulting in a sprained right ankle.
Mill maintenance employee was working on a 14' pipe line and a pump that sanded up. Discharge line slipped out of the upper fitting. In trying to lift the pipe up with the overhead crane, the pipe joint came apart. Pipe came down on employee's hand before he could get it out of the way. Causing fractures and lacerations.
None known - sitting outside of security office when employee was bitten by an unknown insect.
Getting into 420 pit van to go to work. Slipped on walkway rubber mat falling backwards hitting right hip and left shoulder. EE injured shoulder on 9/11/09. She reported it to supervisor at that time. She did not feel she needed medical attention. Shoulder did not seem to be getting better. We took her to the doctor on 9/28/09. She was then put on light duty for up to 2 weeks.
Changing GET on 012 grader. After taking a wear plate off and sitting it on the ground it slipped and the edge cut ee hand between thumb and index finger on left hand. He received 5 stitches it happened on the last hour of the last day worked before scheduled vacation. Will not be back to work until 17th. Stitches come out on the 10th.
Employee was moving the spile remover down the track to the next bar. His right hand was between yoke and cylinder housing. Ram inadvertently retracted smashing right index finger.
Employee was getting loaded under the P&H shovel. The first bucket that was put into the truck had some oversized rock in it. When it hit the bed it jolted the operator of the haul truck hurting her lower back. This was on employees last shift for the week. She was scheduled back on June 12th employee did not make this shift.
