EE was standing on top of a conveyor. He slipped and began to slide down the conveyor on his back. As he was sliding down the conveyor, he tried to slow down the conveyor, he tried to slow his descent by putting his right foot up to brace himself on a metal wind strap. When his foot hit the wind strap, it broke his ankle and he continued sliding.

OUR SERVICE TRUCK WAS STUCK IN THE SAND & EE WENT TO PULL IT OUT WITH A SKID STEER AFTER BEING WARNED NOT TO DO SO. THE TRUCK & THE SKID STEER WERE CONNECTED WITH A TOW STRAP AND IN THE COURSE OF PULLING THE TRUCK OUT, THE STRAP BROKE DUE TO THE TENSION & RECOILED, HITTING EE IN THE FACE. EE SUFFERED A CUT ON LIP AND FRACTURED NOSE.
