The loader operator was standing on the back deck of the front end loader cab cleaning the window. The deck was free from any hazards when he stepped backwards away from the window his foot became entangled in the safety railing causing him to fall first upon the tire and then to the ground breaking his arm.

Showing all 3EE was using his left hand to put pressure on the head of a bolt that was being removed from a piece of metal. The bolt spun and cut through his glove and hand. He went to the local medical center for treatment and will recheck at Medical Center in 7 days.
A rounded sheet of metal was measured for a dozer blade. It was chained to some forks. When it was being lowered to the ground when it was about 18" from the ground it dropped. Because its rounded nature, rather than lay flat on the ground when dropped it rolled over and struch EE on the foot injuring him.
