EE was installing an alternator when he developed leg cramps and as he was dismounting the truck he struck his hand on the fender mirror resulting in a laceration to the Right hand.

The injured was attempting to square off the end of some conveyor belt to make a splice, he was pulling the belt knife toward himself and the knife slipped out of the belt and cut him on the right knee.

Individual was relocating fly pads when a rock fell from the mine roof striking him in the head then while the rock was sliding down his back he received a laceration that required four sutures to close the wound

The injured was bolting in the 7 right crosscut. After he stacked his pusher and finishing piece of steel together and was drilling to his required depth, he went to get a bit out of the tray to change on his starter steel and a piece of rock fell on top of his canopy, and continued to fall on his left hand.

EE was scooping the rib on roadway at the overcast and a roof bolt that was buried in gob, caught scoop bucket and flew back and caught him over the right eye causing a laceration. EE was wearing his safety glasses; the roof bolt broke his safety glasses.

EE was climbing the ladder to hang a chain fall when the ladder slid from underneath him and he fell to the floor hitting his head on a piece of metal that resulted in a laceration to the back of his head.

Employee was working to remove the teeth from an excavator bucket. After cutting the teeth from the bucket EE used a sledge hammer to dislodge the teeth. While pounding the teeth off the bucket a piece of metal broke from the hammer striking his leg. EE was taken by ambulance to Western Maryland Health Systems where he received 5 stitches to repair the laceration.

The injured was operating the bolter to install a fly board for the face of #1. Curtain was hung across the intersection to keep air in the face. After he drilled his hole and was pulling the steel down he used his hand to help bring it out and the air blew the curtain into the driller station catching the steel and wrapping his arm into the steel.

EE was getting a cleanup car, when a rock came out of the conveyor chain of a continuous miner and struck him in the right jaw and behind the right ear. He was x-rayed and determined to have no fractures, but he did have a laceration that required 4 sutures.

While moving a light plant, ee was attempting to lower the tongue of the light plant on the hitch. The tongue slipped off the ball pinching the end of the thumb on his left hand. The injury resulted in a torn fingernail and fracture of the tip of the left thumb.

Employee was taking a guard off of a take-up and while pushing up on the guard, it hit the roof dislodging a piece of draw rock (16" x 16" x 2")that hit employee on the left leg and ankle.

The original date of injury was 4/26/2011, this incident was not classified a reportable incident until 5/17/2011 after a follow up visit to the physician that revealed a fractured bone in his foot and he was put off from work. The injured set the miner up in the # 7 entry and went to get the slack rope off of the machine and a rib rolled on to his foot.

Employee was installing a roof bolt and during the rotation process a small slither of metal off either the bolt or the bolt plate became airborne and struck the employee in the right eye.

While grinding and welding beneath a ramcar tailgate and lying on his back he raised his welding hood and foreign material fell into his left eye. He finished his shift and worked through the next day before continued irritation prompted him to seek medical attention. A piece of material was removed from the eye; he was prescribed medication and then released to unrestricted duty.

While roof bolting along a clay slip a piece of shale fell and struck him on the left foot causing a contusion to the fifth toe. He continued working but after pain persisted he sought treatment at St. Johns ER Springfield where he was diagnosed with a fracture to the toe. After follow up he was released to unrestricted duty.

While roof bolting a drill steel fell against the side of his neck. The steel was hot from use and caused a burn extending behind his right ear. He continued working and finished his shift before seeking medical attention, the wound was cleaned and he was issued a full release. The application of prescription medication made this a reportable injury.

A roof fall was found at 5:40am on 4/4/11 at cross cut 100 entry 4 main south. Fall was 20'wide, 55'long and 12'high. Area had been mined 10/14/1991 and was supported with 5' full resin pins. No injury resulted however 100' of hi-line was lost. There are currently no plans to clean up this fall.

Individual was lifting screen on top the ATRS when his helper pulled on the screen overextending ee shoulder. He was originally seen at Bradous hospital and released to return to work. Due to soreness in the shoulder he saw an Orthopedic surgeon on 4-27-11 and he was placed on light duty until after an MRI can be completed.

23 miners became trapped on the elevator at 6:08 AM when the safety brake on the bottom of the elevator tripped. The elevator was restarted and the men were off the elevator at 6:40 AM. The Shindler elevator team was called and responded to the mine. At 10:37 PM the elevator was cleared for operation.

The fall was approximately 40' long, 10' wide, and fell about 3' high. All approaches have had breaker timbers set and been dangered off.

An unplanned roof fall occurred 2 brks outby spad #6716 in the #8 belt entry outby the #1 section. The fall will be cleaned up according to the fall clean up plan which is posted at all approaches to the fall. The fall dimensions are 17 1/2' W X 50' L X 12' H.

A non-injury un-intentional roof fall occurred (outby) on 1 North Main (return) in #2 entry between cross-cut 16 and 17. The dimensions are approximately 40 feet long, 20 feet wide and 6 feet thick. Rod material is laminated shale. The area was initially supported with 60 inch fully grouted resin rods.

While changing ram batteries, a cylinder lock would not open. He tried hitting the hinge with a hammer. As soon as the hammer struck, the lock sprang open, striking his right thumb. X-rays revealed a non displaced fracture, he was splinted and returned to work without restriction.

The injured and a co-worker were in process of removing plugged coal from the No.7 tail section chute which is part of the raw coal system. While prying with the slate bar to remove the coal, the bar slipped, came up and subsequently struck him on the ear causing a laceration.

While roof bolting and adding a drill extension the extension bowed, sprung free and struck his right hand near the base of the index finger. X-rays revealed a fracture and he was placed on a 'no use of right hand' restriction.

EE was prying a rock off the miner with a slate bar when the rock broke off causing EE's hand and slate bar to slam into the miner smashing his thumb. EE went to the doctor and had an X-ray done which revealed 2 small bone chips on the knuckle of his thumb.

Flint Ridge Mine #2 has experienced a non-injury, unintentional roof fall outby the working section. The fall was discovered on #1 Main at cross-cut 52, #9 entry/intersection on return. The fall is app. 20' long by 20' wide and 5' thick. Laminated shale had deteriorated. The area was initially supported with 48" fully grouted rods.

Employee was using a pry bar to remove a pin from a lift jack on a D-11 Caterpillar Dozer when the bar slipped catching his finger between the bar and the lift jack. Employee was placed on restricted duty for approximately 4 weeks on 03/04/2011.
