While making a weekly return, inspection, the roof fall was discovered in the No. 6 Entry of No. 3 S.E. Mains at SS 1150 3000' outby the active mining section. The area where the fall occurred had previously been dangered off and all entries blocked off leading to it. It doesn't interfere with the ventilation.

Pillars were pulled on the left and right side of bleeder line. Didn't leave enough pillars for support of bleeder line. Fall was inby spad # 1306.
6 Breaks from Main face in #4 portal entry, fall of approx 12" to 19" of loose unconsolidated roof. 2 roof bolts turn out (conventional bolts 36"). Fall approx 21' wide by 15' long. Approx 12,000' outby face area in the intake escapeway, impeding traval. (At spad 38 + 60')
Was making weekly check in return & observed roof fall about 800 feet off section at spad # 1381, 1 break outby. Cribbed and timbered off entries, moved stopping line over to open return air course. Roof fall is about 140 ft long, 20 ft wide x about 15 or 20 ft high.
Rock fell in No. 1 for 220' outby section to 8' approx high entry width beginning in the line of spad 1381 which is in No. 2 entry.
Working on belt with slate bar. Picking up structure, when a piece of rock fell striking person on left side, knocking him down, catching both feet under rock.
Employee was operating the left side of a dual head bolter when his glove tangled in the steel injuring his left hand.
Replacing flights in feeder, cut hand on a burr. Took employee to hospital. Stitched up. Employee returned to work the next day at regular job.
Victim got between scoop and building as operator backed scoop up and steered scoop around.
Found on inspection of old working's rockdust survey inby SS 852. 5'H x 18'W x 40'L. 4' resin bolts.
Began 70' right of spad 1111 extending from # 6 entry to # 8 entry & outby 50' down the # 8 entry roof was supported with 4' resin rods. Laminated shale roof.
Roof fall 5R+to #6+ outby 1 brk. Roof had been supported with 4' resin and 8' cable bolts, laminated shale roof. Fall was 140' long X 20' wide X 8' thick.
