Miner was loosening a bolt on a hydraulic rock drill. The wrench slipped off the bolt, causing the miners arm to hit the drill.

Employee had his left hand on a chain that was connected to a load that had been sat on a trailer. He went to disconnect the chain when the load shifted and the chain pinched his pinky, ring and middle finger on his left hand.

EE was tightening bolts on bucket loader, the breakover bar broke causing the EE to fall backwards. EE broke his fall on his wrist causing injury to his wrist.

Employee was pushing rock. The float on the Bulldozer was engaged. The blade rolled over a large rock, jarring the machine. Employee hit his head on interior window causing an abrasion.

Loader operator was instructed by supervisor to push sand toward screen on flat surface. The operator would push forward, and then back up to start pushing material again. Due to inattention, the operator backed the back tires of the articulated loader sideways on a 5:1 slope unbalancing the machine, and the machine tumbled over onto its side.
