Employee was lancing bridged material from mill feed hopper. They stepped back from the hopper and felt a pop in left knee and felt a sharp pain. Employee was standing on deck grating and did not step on any spillage.

The operator of an off road truck and the Operator of a Excavator were cleaning their windows, when the excavator operator finished EE's windows, EE proceeded to clean out the bed of the truck. The operator of the off road truck was standing on the walk outside of EE's cab and the movement of the truck caused the truck driver to strain EE's back.

Employee was found unresponsive in employee's vehicle in the company parking lot the morning of February 16th, 2022 approx 8:30 am. 911 was called as was MSHA's immediately reportable hotline. Victim was pronounced deceased by the Coroner 9:00 am. The employee passed away of natural causes in the vehicle. An MSHA inspector from the MSHA Macon office performed the investigation.

The injured was attempting to replace a section of conveyor cover. In the process, EE's left hand slipped and EE jammed the middle finger into the cover anchor point, resulting in a laceration of the finger tip.

During a dust collector bag change the miner bent over to pick up a tool. EE's hard hat fell off and EE was struck on top of the head by a falling bag cage. Miner received a laceration requiring three staples.

Employee was getting a pillow-block bearing off the shelf in the warehouse, and EE said felt a pain in the left forearm.

Temp employee was filling bags and upon release the bag fell. Employee attempted to catch the bag but couldn't stop it. EE continued working the shift and complained of mild pain at the end of shift. EE sought medical care on 12/10/18 and was released to return to work. Dr. requested a MRI and results returned on 1/9/19 revealed shoulder injury. Became reportable on 1/9/2019.

Employee was cutting plastic ty-wraps off of a hose. The knife ee was using cut through the ty-wrap unexpectedly, and cut the back of left thumb. Sutures were needed to close the laceration.
Elliott employee was disassembling a machine for repairs. A piece that was believed to be secured unexpectedly moved. Employees right hand was pinched between part and stationary object resulting in a laceration to palm of hand at base of index finger. Cut required nine stitches to close.

Operator misjudgment; heavy dew accumulation; Operator decided to try & turn on the fuel pump by standing on an oil dry container. Operator lost footing & slipped off. The employee worked their entire shift without making any note of injury. Employee later called the foreman over the job to notify the foreman they were at the emergency room getting their back looked at.

The employee was exiting the loader when his right foot slipped off the top step. When his right foot slipped off the step his right knee struck the top step.

To move product to the poly bagger, the operators have to occasionally hit the sides of the bin with a dead blow hammer. While the injured was striking the bin, he said he heard something pop in his shoulder. He was still able to move it freely and refused on-site first aid. He was later diagnosed with a shoulder strain, and his lifting ability was restricted.

On 2/18/14, a temporary contract employee complained of shoulder pain. He could not specify a particular date or time the pain began, but said he thought it was related to his activities at work. The temporary employment agency was contacted and the employee was removed from the worksite.

While working on a pulley taking bolts out employee allegedly got bitten by some type of insect. The insect was never seen. Employee went to the doctor and it was determined the area of the bite needed medical treatment.

Employee was going to check the Silo readings and to see if the elevator was running. When coming back around to bag the employee slipped and fell to the ground. The floor was wet from rain making it slippery. The next morning the employee woke up aching. The employee went to the doctor and it was determined he had bruising of the buttocks.
Employee reported painful & swollen left knee. EE suspects injury was caused by knee repeatedly striking the forklift steering column. Following report of injury EE was observed using improper dismounting technique. Dr report from MRI reveals torn tendon. EE referred to Orthopedic Dr for further rewiew. The company was not notified of accident until 4/2/2012

Stray dogs entered the property. Dogs begin to fight. EE attempted to separate the fighting dogs and was bitten on the left foot. Subsequently EE was ordered to begin rabies vaccination shots.

The driver slipped on clay/mud while trying to hook up his harness. He fell flat on his back and injured his shoulder. Dr.s are calling it a cervical strain.

Employee was working on removing the input shaft from an out of service gearbox. A laceration was caused on right middle knuckle when the bolt he was trying to loosen broke loose and his knuckle struck the shaft coupling.

The employee was weighing 50lb bags. When he stepped backwards, he stepped on an uneven area and lost his balance and fell on the surface he was standing on.
Employee was sweeping up around the bagging machine at a lower level, when he stood up he felt something snap in his lower back. After the investigation, it was determined the employee strained his lower back.

Employee was trying to remove the cover off number eight silo air slide. During this time is when the employee felt a sharp pain in his lower back. Diagnosis, were lumber strain.
Emplyee was operating bulk bag machine when he turned to walk away from the bagging system. He struck his right hand on the I-Beam that supports the bagger and his right knuckle was cut.
