EE was helping move screens to screen deck when her index finger caught on one of the wires of the screen. She ended up with a cut on the tip of her right index finger. She got four stitches, she was not wearing gloves.

Showing all 8There were small amounts of ice on the ground located around the pit. EE was moving belt stand to the location we were going to use it when he slipped on a small patch of ice. He fell to his back and the stand landed on his hip.
EE was holding a 3' bar while I was trying to pound a small pin into the end of the roller for a conveyor, when I hit the pin a small piece of metal split off the pin into the corner of the EE's eye. Had to take him to the doctor to get it removed.
EE was using a chain come-along and it broke causing it to come back and strike him in the head.
EE and supervisor were changing bucket on track hoe. EE was holding 2nd pin when it came out quickly and forced his hand down on the bucket, pinching his left pinkie finger between bucket and pin. EE watched finger over weekend and made Dr appt for 10/28/09. He voluntarily took off Monday and Tuesday.
He was lifting a cutting edge to be mounted on loader bucket. Lifted improperly and hurt lower back.
He was working on a gear box. He had to stand on a tail pulley guard to reach it, which is about 3.5 ft off the ground. While working on the gearbox he was bringing up a wrench and slipped as he was climbing the tail pulley guard. He landed on a rock as he fell which caused him to loose his balance and fall on another rock on his hip.
Ducked under conveyor belt and stood up to fast and bumped head. He must have twisted, sending pain though right shoulder, neck and arm.
